Abstract Statement of problem Direct (intraoral) and indirect (desktop) digital scanning can record abutment tooth preparations despite their geometry. However, little peer-reviewed information is ...available regarding the influence of abutment tooth geometry on the accuracy of digital methods of obtaining dental impressions. Purpose The purpose of this in vitro study was to evaluate the influence of abutment tooth geometry on the accuracy of conventional and digital methods of obtaining dental impressions in terms of trueness and precision. Material and methods Crown preparations with known total occlusal convergence (TOC) angles (−8, −6, −4, 0, 4, 8, 12, 16, and 22 degrees) were digitally created from a maxillary left central incisor and printed in acrylic resin. Each of these 9 reference models was scanned with a highly accurate reference scanner and saved in stereolithography (STL) format. Then, 5 conventional polyvinyl siloxane (PVS) impressions were made from each reference model, which was poured with Type IV dental stone scanned using both the reference scanner (group PVS) and the desktop scanner and exported as STL files. Additionally, direct digital impressions (intraoral group) of the reference models were made, and the STL files were exported. The STL files from the impressions obtained were compared with the original geometry of the reference model (trueness) and within each test group (precision). Data were analyzed using 2-way ANOVA with the post hoc least significant difference test (α=.05). Results Overall trueness values were 19.1 μm (intraoral scanner group), 23.5 μm (desktop group), and 26.2 μm (PVS group), whereas overall precision values were 11.9 μm (intraoral), 18.0 μm (PVS), and 20.7 μm (desktop). Simple main effects analysis showed that impressions made with the intraoral scanner were significantly more accurate than those of the PVS and desktop groups when the TOC angle was less than 8 degrees ( P <.05). Also, a statistically significant interaction was found between the effects of the type of impression and the TOC angle on the precision of single-tooth dental impressions (F=2.43, P =.002). Visual analysis revealed that the intraoral scanner group showed a homogeneous deviation pattern across all TOC angles tested, whereas scans from the PVS and desktop scanner groups showed marked local deviations when undercuts (negative angles) were present. Conclusions Conventional dental impressions alone or those further digitized with an extraoral digital scanner cannot reliably reproduce abutment tooth preparations when the TOC angle is close to 0 degrees. In contrast, digital impressions made with intraoral scanning can accurately record abutment tooth preparations independently of their geometry.

Temporal summation of second pain (TSSP) has been suggested as a psychophysical index for central sensitization, one of the critical mechanisms in the chronification of pain. However, there ...is no gold standard for protocols to measure TSSP. The purpose was to establish the stimulus intensity for measuring TSSP. Female patients with chronic myofascial temporomandibular disorders pain (n = 16) and healthy female volunteers with no pain (n = 15) participated. Pain thresholds (PT °C) were measured, and repetitive heat stimuli at three stimulus intensities (PT °C, PT + 1 °C, PT + 2 °C) were applied. TSSP parameters were quantified as TSSP magnitude (TSm) and TSSP frequency (TSf). In healthy female volunteers, pain ratings significantly decreased at PT °C (p < 0.050), besides TSm and TSf at PT + 2 °C were significantly higher than those at PT °C (p < 0.025). In chronic pain patients, pain ratings significantly increased at PT + 1 °C and PT + 2 °C (p < 0.050). At PT + 2 °C, TSm and TSf in chronic pain patients were significantly higher than those in healthy volunteers (p < 0.050). It could be helpful to measure TSSP with the stimulus intensity adjusted individually to the patient’s pain thresholds + 2 °C for assessing central sensitization.
Abstract Objectives Occlusal overloading is one of the causes of peri-implant bone resorption, and many studies on stress distribution in the peri-implant bone by three-dimensional finite element ...analysis (3D FEA) have been performed. However, the FEA models previously reported were simplified and far from representing what occurs in clinical situations. In this study, 3D FEA was conducted with simulation of the complex structure of dental implants, and the influences of neck design and connections with an abutment on peri-implant bone stress and abutment micromovement were investigated. Methods Three types of two-piece implant CAD models were designed: external joint with a conical tapered neck (EJ), internal joint with a straight neck (IJ), and conical joint with a reverse conical neck (CJ). 3D FEA was performed with the setting of a “contact” condition at the component interface, and stress distribution in the peri-implant bone and abutment micromovement were analyzed. Results The shear stress was concentrated on the mesiodistal side of the cortical bone for EJ. EJ had the largest amount of abutment micromovement. While the von Mises and shear stresses around the implant neck were concentrated on the labial bone for IJ, they were distributed on the mesiodistal side of the cortical bone for CJ. CJ had the least amount of abutment micromovement. Significance Implants with a conical joint with an abutment and reverse conical neck design may effectively control occlusal overloading on the labial bone and abutment micromovement.
The effect of cleaner containing 10-methacryloyloxydecyl dihydrogen phosphate (MDP) for removing temporary cement remnants on dentin surface was evaluated. Flat dentin surfaces were wet-polished (Co) ...and HY-BOND temporary cement hard (Shofu) was applied to the surface. This temporary cement was removed using an air-scaler (Sc), brush (Br), or phosphoric acid and NaOCl (NC). A prototype cleaner containing MDP (Kuraray Noritake Dental, Tokyo, Japan) was used with agitation mode (MC+AG). KATANA Avencia block (Kuraray Noritake Dental) was luted with SA Cement Plus Automix (Kuraray Noritake Dental). Co showed significantly higher bond strength than Sc or Br (p<0.001 each). Bond strengths with NC (p=0.99) and MC+AG (p=0.38) did not differ significantly from that with Co. Transmission electron microscopy revealed sufficient interaction of MC+AG. Cleaner containing MDP can effectively remove temporary cement by agitation, and can be expected to improve the chemical bonding ability by binding more MDP to dentin.
The effects of silanization and resin primer application on CAD/CAM indirect resin composite block bonding were investigated. KATANA AVENCIA P blocks (Kuraray Noritake Dental) were treated with a ...silane coupling agent and/or a resin primer. The contact angles (CAs) of resin primer were observed before and after silanization. Panavia V5 (Kuraray Noritake Dental) was built after each treatment. Bond strengths were measured, and the interface was analyzed by energy dispersive X-ray spectroscopy (EDS). The CA showed that silane treatment improved the wettability of the resin primer to the resin block. The combination treatment of the silane and resin primer showed significantly higher bond strength than no treatment, only in the silanization or resin primer group (p<0.001 each). EDS analysis showed that the resin primer penetrated both cement and block sides. The combination of the silane and resin primer improved bonding effectiveness between the resin block and resin cement.
Nitrogen-containing bisphosphonates such as zoledronic acid (ZOL) and pamidronate have been widely and successfully used for the treatment of cancer patients with bone metastases and/or ...hypercalcemia. Accumulating recent reports have shown that cancer patients who have received these bisphosphonates occasionally manifest bisphosphonate-related osteonecrosis of the jaw (BRONJ) following dental treatments, including tooth extraction. However, little is known about the pathogenesis of BRONJ to date. Here, to understand the underlying pathogenesis of BRONJ, we examined the effects of ZOL on wound healing of the tooth extraction socket using a mouse tooth extraction model. Histomorphometrical analysis revealed that the amount of new bone and the numbers of blood vessels in the socket were significantly decreased in ZOL-treated mice compared to control mice. Consistent with these results, ZOL significantly inhibited angiogenesis induced by vascular endothelial growth factor in vivo and the proliferation of endothelial cells in culture in a dose-dependent manner. In contrast, etidronate, a non-nitrogen-containing bisphosphonate, showed no effects on osteogenesis and angiogenesis in the socket. ZOL also suppressed the migration of oral epithelial cells, which is a crucial step for tooth socket closure. In addition, ZOL promoted the adherence of
Streptococcus mutans
to hydroxyapatite and the proliferation of oral bacteria obtained from healthy individuals, suggesting that ZOL may increase the bacterial infection. In conclusion, our data suggest that ZOL delays wound healing of the tooth extraction socket by inhibiting osteogenesis and angiogenesis. Our data also suggest that ZOL alters oral bacterial behaviors. These actions of ZOL may be relevant to the pathogenesis of BRONJ.

This study aimed to evaluate the influence of tooth and adhesive temperature during the bonding procedure on the effectiveness of dentin bonding. The results showed that the tooth temperature affected the effectiveness of the dentin bonding; therefore, to determine the precise bonding ability in the laboratory, the temperature of the tooth must be raised until it is the same as that of the oral cavity. In addition, the temperature of the material did not affect bonding effectiveness; this result confirms that it does not matter whether the refrigerated product is used soon after its removal from the refrigerator or after it reaches room temperature in the clinic.
The resin-coating technique is one of the successful bonding techniques used for the indirect restorations. The dentin surfaces exposed after cavity preparation are coated with a thin film of a ...coating material or a dentin bonding system combined with a flowable composite resin. Resin coating can minimize pulp irritation and improve the bond strength between a resin cement and tooth structures. The technique can also be applied to endodontically treated teeth, resulting in prevention of coronal leakage of the restorations. Application of a resin coating to root surface provides the additional benefit of preventing root caries in elderly patients. Therefore, the coating materials have the potential to reinforce sound tooth (“Super Tooth” formation), leading to preservation of maximum tooth structures.
The fluorescence and physical properties of thulium-doped zirconia were investigated. A standard grade of zirconia (TZ-3Y-E) and two translucent dental zirconia materials (Zpex and Zpex Smile) were ...examined. The specimens were prepared by addition of 0–1.5 wt% Tm2O3 and then sintering. When exposed to UV light, the Tm2O3-doped zirconia exhibited blue fluorescence with a peak wavelength of 460 nm. The fluorescence intensity of Zpex and Zpex Smile was higher than that of TZ-3Y-E, with Zpex being more intense than Zpex Smile. Zpex exhibited maximum fluorescence intensity when doped with 0.8 wt% Tm2O3. XRD analysis revealed that TZ-3Y-E and Zpex contained primarily tetragonal zirconia, while Zpex Smile contained largely cubic phase zirconia. There were no changes observed in the microstructure or physical properties of the zirconia specimens when doped with Tm2O3.
