Purpose
LHON is an orphan mitochondrial disorder affecting the retinal ganglion cells leading to permanent blindness from which recovery is rare. An increasing body of evidence indicates that ...idebenone has therapeutic potential for the treatment of LHON. Data from a randomized placebo‐controlled study (RHODOS), from a number of case reports and retrospective cohort studies demonstrate that patients with established vision loss may benefit from idebenone treatment and recover visual acuity (VA).
This study reports VA outcomes for patients with recent onset of vision loss who received idebenone treatment under an ongoing global Expanded Access Program (EAP) where currently 82 patients are enrolled and have been treated for an average of 15 months. The outcomes will be compared with findings of the RHODOS study and a Case Record Survey collecting VA data from untreated patients.
Methods
Analyses are performed to assess recovery from the VA nadir. Clinically relevant recovery was defined as (i) improvement from nadir by at least 10 letters on the ETDRS chart or (ii) improvement from “off‐chart” at nadir to being able to read at least 5 letters on‐chart. Furthermore, the prevention of vision loss for patients with residual vision below 1.0 logMAR (20/200) at start of therapy was analysed.
Results
A high proportion of patients (about 50% at submission of the ) treated with idebenone under this global EAP experienced a clinically meaningful recovery of vision. In addition, in patients with residual vision below 1.0 logMAR at start of therapy, loss of VA to above this level could be prevented in a large number of patients (about 60% at submission of the ).
Conclusions
The therapeutic potential of idebenone in the treatment of LHON is further demonstrated by the clinical experience in a large cohort of patients under a global EAP.
Financial aid for the blind which is awarded based only on medical certificates and results of examinations has in the past resulted in too many false diagnoses; therefore, Bavaria seeks to pay ...financial aid to the blind only on the basis of a specific ophthalmological assessment according to the standards of the German pension medical ordinance (VersMedV, Versorgungsmedizinische Verordnung). Because these ophthalmological assessments initially contribute to a higher financial burden on the state, longer processing times and inconvenience to the patient, investigations should be undertaken to determine if ophthalmological findings, reports and medical certificates can be a suitable basis for an expert assessment and in how many cases blindness which had been certified by the original examination could be confirmed by a specific ophthalmological assessment.
A total of 925 applications for financial assistance to the blind within the catchment area of the Bavarian Center for Family and Social Services (ZBFS, Zentrum Bayern Familie und Soziales) regional center in Upper Bavaria between 2003 and 2008, all of which had been subjected to an assessment by the same practitioner acting as external expert, were statistically analyzed.
Of the 357 applicants who had been classified as blind according to the medical certificate and findings, 283 (79 %) were confirmed as being blind after the assessment and in 73 (21 %) blindness could not be confirmed. Of the 262 applicants who were classified as not being blind during the first examination, the diagnosis was confirmed in 192 (73 %) while 70 cases (27 %) were classified as blind. Of the 304 applicants for whom an assessment was not possible by the medical certification, 165 were ultimately classified as blind and 139 as not blind. Out of 32 applicants who were explicitly classified as being blind in the medical certificate, 13 were confirmed as being blind while the remaining 18 were classified in the subsequent assessment as not blind.
Apart from unambiguous cases, the awarding of financial aid to the blind should only be based on an ophthalmological assessment which follows the VersMedV guidelines. Only this approach can result in an equal treatment of all applicants before the law and the awarding of financial aid to the blind to assist those truly in need. In addition, fiscal budget results revealed that the falsely awarded financial aid to the blind reached well beyond millions of Euros.
Due to demographic change and societal transformation the number of elderly persons living in retirement homes is growing in Germany. Access to health care is more complicated in the setting of ...nursing homes. Different regional studies suggest unmet ophthalmological health care needs in institutionalized elderly people. This study assessed the current ophthalmological health care structure and supply status in nursing homes in Germany.
This prospective, multicenter cross-sectional study was conducted by 14 study centers in Germany. Elderly people living in 32 nursing homes were included after approval by the local institutional review boards. A standardized examination was performed which included a detailed medical and ocular history, refraction, visual acuity testing, tonometry, biomicroscopy and dilated funduscopy. Unmet ophthalmological health care needs were documented and the data were analyzed descriptively and via logistic regression modelling.
A total of 600 participants (434 women and 166 men) aged 50-104 years were examined of which 368 (61%) had ophthalmological conditions requiring treatment. The most prevalent findings were cataracts (315; 53%), disorders of the eyelids (127; 21%), dry eye disease (57; 10%) and posterior capsule opacification (43; 7%). In 63 (11%) of the participants glaucoma was suspected and 55 (9%) of the examined population had a known diagnosis of glaucoma, of whom one third was not on any or on insufficient anti-glaucomatous therapy. 236 (39%) showed signs of age-related macular degeneration (AMD). Only 52% of the examined cohort had been examined by an ophthalmologist within the last 5 years and 39% stated that they would currently not be able to consult an ophthalmologist. Reported barriers were mainly transport and lack of support.
This study demonstrates considerable unmet ophthalmological health care needs of the institutionalized elderly in Germany. Novel and reformed models of specialist care provision have to be developed.

Here we report visual acuity outcomes for patients ...with recent onset who received Raxone® (idebenone) under an ongoing global Expanded Access Program (EAP).
Methods Visual acuity was measured in 3‐monthly intervals. Clinically relevant recovery was defined as (i) improvement from nadir by at least 10 letters on the ETDRS chart or (ii) improvement from “off‐chart” at nadir to being able to read at least 5 letters on‐chart.
Results Currently there are 61 LHON patients enrolled, of which 48 patients have been treated for an average of 11 months. So far, 24 of 48 patients (50%) have experienced a clinically relevant and stable recovery in VA (89% for T14484C, 70% for G3460A and 31% for G11778A). The average treatment effect size in patients with recovery was 29 letters and 84 % recovered within 12 months of the initiation of the EAP.
Conclusion A high proportion of LHON patients treated with idebenone under a global EAP experienced a clinically meaningful recovery of vision, further demonstrating the therapeutic potential of idebenone in the treatment of LHON.

Ziel der vorgestellten SK2-Leitlinie gemäß den Regeln der Arbeitsgemeinschaft Wissenschaftlich Medizinischer Fachgesellschaften ist es die Begrifflichkeit, inkl. der damit verbundenen Konzepte, zu diskutieren, Ätiologien der Störung und daraus resultierende Risikogruppen, die einer systematischen Untersuchung zugeführt werden sollten, zu benennen und letztlich eine Orientierung über die erforderliche ophthalmologische, kinderneurologische und neuropsychologische Diagnostik zu geben und die therapeutischen und sonderpädagogischen Möglichkeiten und Anforderungen darzustellen.

Hintergrund und Ziel der Arbeit
Da die Bewilligung von Blindengeld anhand von Attesten und Befundberichten in der Vergangenheit zu vielen Fehlentscheidungen geführt hatte, wird in ...Bayern angestrebt, Blindengeld nur noch auf Grundlage einer augenärztlichen Begutachtung zu zahlen. Da dies jedoch zusätzliche Kosten verursacht, zu einer längeren Bearbeitungsdauer und zu einer zusätzlichen Belastung der Patienten führt, sollte mit dieser Studie festgestellt werden, ob dieses Vorgehen dennoch gerechtfertigt werden kann. Außerdem sollte untersucht werden, ob sich die höheren Kosten für augenärztliche Gutachten durch die Vermeidung von zu Unrecht gezahlten Leistungen amortisieren.
Methodik
Statistisch ausgewertet wurden 925 Blindengeldanträge aus dem Einzugsgebiet des Zentrums Bayern Familie und Soziales (ZBFS), Regionalstelle Oberbayern, aus den Jahren 2003 bis 2008, die alle von dem gleichen als Außengutachter tätigen niedergelassenen Augenarzt begutachtet wurden.
Ergebnisse
Bei 357 Antragstellern, die anhand der Befunde und Atteste als „blind“ eingestuft wurden, wurde nach der ersten Begutachtung in 283 Fällen (79 %) Blindheit bejaht, in 73 Fällen (21 %) konnte Blindheit nicht bestätigt werden. Bei 262 nach den Attesten als „nicht blind“ eingestuften Antragstellern wurde diese Beurteilung bei der Begutachtung in 192 Fällen (73 %) bestätigt, in 70 Fällen (27 %) wurde dagegen Blindheit festgestellt. Von den 304 Antragstellern, bei denen nach den Attesten keine Beurteilung möglich war, wurden letztlich 165 als „blind“ und 139 als „nicht blind“ eingestuft. Bei insgesamt 32 Antragstellern, die im Attest expressis verbis als „blind“ bezeichnet worden waren, wurde in 13 Fällen Blindheit bestätigt, die anderen 18 waren im anschließenden Gutachten als „nicht blind“ eingestuft worden.
Diskussion
Eine Bewilligung von Blindengeld sollte nur auf Grundlage einer augenärztlichen Begutachtung unter Beachtung der Richtlinien der VersMedV erfolgen. Nur so kann eine Auszahlung des Blindengeldes an die tatsächlich Betroffenen gewährleistet werden. Diese Vorgehensweise ist im Interesse einer Gleichbehandlung aller Antragsteller vor dem Gesetz wünschenswert. Zudem ergeben sich für den Fiskus über die Jahre hinweg durch sonst zu Unrecht gezahltes Blindengeld Kosteneinsparungen in Höhe mehrstelliger Millionenbeträge.

Hintergrund und Fragestellung
Aufgrund der demografischen und gesellschaftlichen Entwicklung ist die Anzahl der Menschen höheren Lebensalters gestiegen, die in Seniorenheimen ...vollstationär gepflegt werden. Mit dem Wechsel in ein Heim wird die ärztliche Versorgung komplizierter; verschiedene regionale Studien weisen insbesondere auf eine ambulante fachärztliche Unterversorgung hin. Vor diesem Hintergrund wurde in dieser Studie die aktuelle ophthalmologische Versorgung in Seniorenheimen in Deutschland untersucht.
Methoden
Diese prospektive, multizentrische Querschnittstudie wurde durch 14 Studienzentren in Deutschland im Zeitraum von 2014 bis 2016 durchgeführt. Insgesamt wurden Bewohner von 32 Einrichtungen nach Genehmigungen durch die zuständigen Ethikkommissionen untersucht. Die standardisierte augenärztliche Untersuchung beinhaltete Anamnese, Refraktion, Visusbestimmung, Tonometrie, Spaltlampenuntersuchung und Funduskopie in Mydriasis. Alle Untersuchungen fanden jeweils vor Ort in den Heimen statt. Der augenärztliche Behandlungsbedarf wurde charakterisiert, und die Daten wurden deskriptiv und mittels logistischer Regression ausgewertet.
Ergebnisse
Insgesamt wurden 600 Bewohner (434 Frauen und 166 Männer) zwischen 50 und 104 Jahren untersucht. Für 368 Personen (61%) ergab sich ein ophthalmologischer Befund, der eine weitere Behandlung erforderte. Zu den häufigsten Diagnosen des Vorderabschnitts zählten eine visusrelevante Katarakt (315; 53%), Veränderungen der Augenlider (127; 21%), Benetzungsstörungen (57; 10%) und Nachstar (43; 7%). Bei 63 (11%) Bewohnern zeigte sich ein Glaukomverdacht, und bei 55 (9%) bestätigte sich die Diagnose eines vorbekannten Glaukoms, welches jedoch bei einem Drittel unzureichend oder gar nicht therapiert wurde. Funduskopisch zeigten sich bei 236 Bewohnern (39%) Zeichen einer altersabhängigen Makuladegeneration (AMD). Lediglich 52% der Senioren waren innerhalb der letzten 5 Jahre augenärztlich untersucht worden. 39% der Bewohner gaben an, dass sie sich aktuell nicht in der Lage sehen würden, einen Augenarzt zu konsultieren. Insgesamt wurden 3,7% der untersuchten Bewohner von den Studienärzten als bettlägerig eingestuft. Berichtete Hürden waren vorwiegend Transport und fehlende Unterstützung/Begleitung.
Schlussfolgerung
Die vorliegende Erhebung zeigt ein erhebliches ophthalmologisches Versorgungsdefizit in deutschen Seniorenheimen. Neue bzw. eine Erweiterung derzeitiger Versorgungsmodelle sind dringend notwendig.
