Introduction. Besides participation in the primary prevention, screening as secondary prevention is an important requirement for primary care services. The effect of this work is influenced by the ...characteristics of individual primary care practices and doctors’ screening habits, as well as by the regulation of screening processes and available financial resources. Between 1999 and 2009, a managed care program was introduced and carried out in Hungary, financed by the government. This financial support and motivation gave the opportunity to increase the number of screenings.
Method. 4,462 patients of 40 primary care practices were screened on the basis of SCORE risk assessment. The results of the screening were compared on the basis of two groups of patients, namely: those who had been pre-screened (pre-screening method) for known risk factors in their medical history (smoking, BMI, age, family cardiovascular history), and those randomly screened. The authors also compared the mortality data of participating primary care practices with the regional and national data.
Results. The average score was significantly higher in the pre-screened group of patients, regardless of whether the risk factors were considered one by one or in combination. Mortality was significantly lower in the participating primary practices than had been expected on the basis of the national mortality data.
Conclusion. This government-financed program was a big step forward to establish a proper screening method within Hungarian primary care. Performing cardiovascular screening of a selected target group is presumably more appropriate than screening within a randomly selected population. Both methods resulted in a visible improvement in regional mortality data, though it is very likely that with pre-screening a more cost-effective selection for screening may be obtained.
Uvod. Poleg primarne preventive je presejanje kot sekundarna preventiva pomemben člen pri storitvah osnovnega zdravstvenega varstva. Na uspešnost tovrstnega dela vplivajo značilnosti posameznih splošnih ambulant varstva in pripravljenost zdravnikov za izvajanje presejalnih pregledov kot tudi ureditev procesov presejanja in razpoložljivih finančnih virov. Med leti 1999 in 2009 je bil v Madžarski uveden in izpeljan program vodene oskrbe, ki ga je financirala vlada. Ta finančna podpora in spodbuda je omogočila priložnost za povečanje števila presejalnih pregledov.
Metode. 4462 pacientov iz 40 splošnih ambulant je bilo vključenih v presejalni pregled v sklopu ocene tveganja SCORE. Rezultate presejanja se je primerjalo na podlagi dveh skupin pacientov, in sicer tistih, ki so bili predhodno presejani (metoda predhodnega presejanja) za znane dejavnike tveganja v njihovi zdravstveni anamnezi (kajenje, indeks telesne mase, starost, zgodovina srčno-žilnih obolenj) ter tistih, ki so bili presejani naključno. Avtorji so primerjali tudi podatke sodelujočih splošnih ambulant o smrtnosti z regionalnimi in nacionalnimi podatki.
Rezultati. Povprečen rezultat je bil bistveno višji v predhodno presejani skupini pacientov, ne glede na to, ali so bili dejavniki tveganja upoštevani posamično ali v kombinaciji. Smrtnost je bila bistveno nižja pri sodelujočih splošnih ambulantah, kot je bilo pričakovati na podlagi nacionalnih podatkov o smrtnosti.
Zaključek. Ta program, ki ga je financirala država, je pomenil velik korak naprej k ustanovitvi ustrezne metode presejanja znotraj madžarskega sistema osnovne zdravstvene oskrbe. Izvajanje presejanja za ogroženost srčnožilnega sistema pri izbrani skupini je očitno bolj primerno od naključnih pregledov. Obe metodi sta vidno prispevali k izboljšanju regionalnih podatkov o smrtnosti, čeprav je precej verjetno, da se s predhodnim presejanjem doseže bolj stroškovno učinkovita izbira presejanj.

Correlations between obesity and asthma control in children: Hungarian primary care pilot study Introduction: Asthma is often associated with overweight and obesity. The aim of this study was to find ...associations between asthma control, obesity and different levels of physical activity. Methods: Using a questionnaire, 117 asthmatic patients between 6-18 years of age were interviewed. Two groups of children, normal vs. overweight, at different levels of physical activity (sport, school-based proper or light activity and full physical exemption) were compared. Asthma control was evaluated in two groups (controlled vs. non or partially controlled). Results: The asthmatic status was generally controlled in 78%; in 81% and 72% of patients with a normal weight and overweight, respectively. Being overweight was more common among girls than boys (43.2% vs. 30.3%). A positive family history of asthma was revealed in many cases; the prevalence of asthma was found at 63% (P=0.0074) among the parents. Weight was significantly higher when the parents themselves were overweight. A lightened workload in physical education lessons at school doubled the risk of obesity (P=0.25), while full exemption increased it by six times (P=0.06). High bodyweight was found in 37% and 31% of children who had received steroid medication and other treatment, respectively (P=0.57). Conclusion: High bodyweight and physical inactivity worsened the chances of effective asthma treatment, while sport improved it. The rates of physical activity among the surveyed patients were lower than recommended. More focus is needed; paediatricians, school-teachers and parents should pay more attention to the issue when establishing a proper family background for healthier lifestyles. PUBLICATION ABSTRACT

The fifty years or so preceding the watershed of 1848–49 witnessed the emergence of liberal nationalism in Hungary, along with a transmutation of conservatism which appeared then as a party and an ...ideological system in the political arena. The specific features of the conservatism, combining the protection of the status quo with some reform measures, its strategic vision, conceptual system, argumentation, assessment criteria and values require an in depth exploration and analysis. Different conservative groups were in the background or in opposition from 1848 to 1918, while in the period between the two World Wars, they constituted the overwhelming majority of ruling parties. During the one-party system, from 1949 to 1989, the liberals and conservatives—like all other political groups—were illegal, a status from which they could later emerge upon the change of the political system. The inheritance of the autocratic system frozen up and undigested by the one-party state was thawed after the peaceful regime change, the constitutional revolution and its discrete components began to be reactivated, including the enemy images of earlier discourses. “Liberal” and “conservative” had become state-party stigmas in line with fascist, reactionary, rightist, and bourgeois. In reaction to that, at first conservative then liberal, intellectual fashions and renascences unfolded in the 1980s. The attempts by liberal and conservative advocates to find predecessors did not favor an objective approach. The first step toward objectivity is establishing distance from the different kinds of enemy images and their political idioms. This is a pressing need because, although several pioneering works have appeared on different variants of the Hungarian liberalisms and conservatisms, there are no serious unbiased syntheses. This work is urgent because the political poles of the constitutional revolution and the ensuing period have up till now been described in terms of different conspiracy theories.
Before the publication of this book, Alaine Polcz was widely recognized as a psychologist ministering to the needs of disturbed and incurably ill children and their families, as the author of ...numerous articles and several books on thanatology, and as the founder of the hospice movement in Hungary. The autobiographic account of the experiences of a woman, then 19-20, in the closing months of the Second World War. When it was first published, in 1991, the book was a revelation of past horrors in Hungary which, until then, had lingered on in the farthest reaches of the national memory as rumor and suspicion about the violent acts committed against women during a time of chaos, havoc, and savagery. The literary world quickly recognized the merits of this book: It was highly praised by Hungarian reviewers, awarded prizes, and has already been translated into French, Rumanian, Slovenian, and Serbian.
