...日本救急医学会ではこれまで熱中症発生に関する調査を行ってきた。2016年と2017年に,熱中症による入院患者と外国人,身体障害者を対象として熱中症発生調査を行った。【目的】本邦における熱中症の発生について調査することである。【方法】全国の二次・三次救急指定病院を対象としてFaxによる即日登録によって行った。【対象】熱中症を主病名として入院した症例とした。同時に2020年に東京オリンピック・パラリンピック開催を踏まえて,外国人と身体障害者も対象とし調査を行った。【結果】登録施設数は,HSF 2016では163施設,HSF 2017では134施設であった。登録症例数は,それぞれ843例と713例であり,記載不備などを除外すると778例と626例となった。登録された身体障害者は,HSF 2016では37例,HSF 2017では20例であった。外国人は,それぞれ4例と2例という結果であった。両期間ともに男性が多く,年齢は11歳台と61歳以上にピークを認めた。高齢者では,屋内発生が屋外発生を上回っていた。重症度別では,III度が最も多く認められた。【考察】入院患者が対象のため,III度熱中症が多く認められた。男性に多く,11歳台の若年者と61歳以上の高齢者に多く,高齢者では屋内発生が多い点は過去の調査と同様の結果であった。身体障害者と外国人の調査については登録症例数が少なく今後の調査の課題となった。
ABSTRACT
Since 2006, the Japanese Association for Acute Medicine has conducted a survey on the occurrence of heat related illness in emergency designated hospitals nationwide. In 2016 and 2017, we report on the severe heat related illness and an immediate registration study on the occurrence of heat related illness in foreign and disabled people.
Objective: To investigate the occurrence of heat related illness in Japan.
Method: These was carried out by the same day registration by fax for secondary and tertiary emergency designated hospitals nationwide.
Target: A patient was admitted to our hospital with heat related illness as the main disease name. Based on the Tokyo Olympic Games and Paralympic Games held in 2020, we also conducted surveys targeting foreigners and disabled people.
Results: The number of registered facilities was 163 in HSF 2016 and 134 in HSF 2017. The number of cases registered was 843 cases and 713 cases respectively, and when excluded such as incomplete description, it was 778 cases and 626 cases. The number of registered physically handicapped was 37 cases at 2016 and 20 cases at 2017. Foreigners were 4 cases and 2 cases respectively. There were many men in both periods, and the age peaked in the 11 years old and 61 years old and over. In the elderly, indoor generation exceeded outdoor occurrence. By severity level, III degree was the most frequent.
Discussion: Because it was targeted in inpatients, severe heat related illness was frequent. There were many in men, young people aged 11 years old and older people aged 61 and older, and the elderly had many indoor occurrences similar to the previous survey. In parallel, we conducted surveys of people with disabilities and foreigners, but the number of registered cases was small and it was a subject of future survey.
Conclusion: In 2016 and 2017 surveillance was conducted for second–degree and tertiary emergency designated medical institutions for hospitalized patients.
An effective method based on the Genetic Algorithms is proposed to solve the Handicapped Person Transportation problem, which is a real-life application for pickup and delivery problems. In these ...problems, vehicles have to transport (clients, loads, etc.,) from their locations to different destinations (hospitals, shop centres, etc.). The objective of this paper is to implement Grouping Genetic Algorithm to find optimal (or close to optimal) routes for transporting handicapped people in terms of service quality and number of used vehicles. This algorithm is a stochastic search method based on randomized operators for combining solutions and producing better ones. The proposed algorithm has been applied on the handicapped persons transportation problem in the city of Brussels, Belgium. The obtained results are better than the manually generated solutions in terms of service quality and computational effort.
Handicapped persons who spilled food unconsciously were studied with the aim to investigate the spilling conditions related to the spoon angles (rotational and azimuthal angles) and the inclination ...angle of the liquid food surface estimated from the acceleration of the spoon when used by a handicapped person to eat food. Image captures of eating behaviors were recorded from three directions using three cameras, with markers attached to the wrist of the subject and to the spoon. Both the position and the acceleration of the spoon measured by three-dimensional (3D) image processing were investigated to determine the spilling conditions. Spilling of liquid food was observed at a rotational angle of -18 deg and an azimuthal angle of +16 deg at the start of the spoon motion. Thus, we predicted that spilling of liquid food during spoon movement was affected not only by the spoon angle but also by the spoon acceleration speed. Evaluation of spilling conditions using a two-dimensional spoon model (L60×W36×D7 mm, liquid food filled to 40% of the spoon depth) showed that spill could be avoided at rotational angles in the range of ±17 deg and at azimuthal angles in the range of +8 to -13 deg. When the liquid food inclination angle was estimated from measured spoon acceleration speed, we found that during actual eating the liquid food was inclined at angles within the spilling condition. Theses results suggest that the spilling of liquid food during spoon movement is related not only to the spoon angle, but also to the inclination angle of the liquid surface induced by spoon acceleration.

During their walking time, naturally handicapped persons have difficulty identifying obstructions in front of them, making it extremely unsafe. The smart stick is designed to help kids recognize the ...environment around them. The obstruction is not detected in normal stick, and it is ineffectual for vision impaired people. Because a blind individual has no idea what kinds of things or objects are in front of him. The application of Arduino on an ultrasonic blind walking stick will be explained in this study. According to the World Health Organization, 30 million people are irreversibly blind, with 285 billion people visually handicapped. When you take those persons, you will quickly see that they cannot walk to their destination without the assistance of others, and that they must ask for instructions. They are confronted with additional problems in their daily lives. This blind stick makes walking incredibly safe for someone who can't see. This stick detects an item in front of the user and responds to the user by vibrating or issuing a command, allowing the man to navigate without fear. This device will be the most effective way for them to overcome their issues. A person can walk more confidently with the help of this blind stick. We're planning to improve the project by expanding its scope. The Arduino embedded system, vibration motor, ISD1932 flash memory, and two ultrasonic sensors will all be used in this project. Now, this smart stick will contain an ultrasonic sensor to detect distance from any object, as well as an RF remote for the blind guy to locate his stick remotely. The stick detects all obstacles within a 4 meter range in 39 milliseconds and sends a corresponding respect message, allowing the blind to walk twice as fast as usual since she/he feels secure.
成人後に発症する先天代謝異常症(inborn errors of metabolism, ...IEM)はまれと考えられているが,詳細は不明である.2001年から2010年に当科で,成人後に診断されたIEMについて臨床経過や予後を検討した.IEMと診断した386例のうち24例が成人後に診断された.そのうち成人期の発症例は15例あった.疾患の内訳はアルカプトン尿症11例,その他の有機酸代謝異常症6例,尿素サイクル異常症4例,脂肪酸代謝異常症3例であった.IEMの成人例はまれであるが,発症時期にかかわらず,診断の遅れが患者QOLの低下に繋がる.原因不明の発達遅滞や退行例などでは,成人であってもIEMを念頭においた検索が必要である.
The lack of carers for senior citizens and physically handicapped persons in our country has now become a huge issue and has created a great need for carer robots. The usual carer robots (many of ...which have switches or joysticks for their interfaces), however, are neither easy to use it nor very popular. Therefore, haptic devices have been adopted for a human-machine interface that will enable an intuitive operation. At this point, a method is being tested that seeks to prevent a wrong operation from occurring from the user's signals. This method matches motions with EMG signals.
Dietary reference intakes (DRIs) for the Japanese target healthy individuals and groups. However, DRIs are used for nutrition planning in support institutions for handicapped persons, because there ...is insufficient data to clarify the amount of energy and nutrition necessary for persons with motor disability. This present study concerns a dietary survey and physical investigation of six men (54.0±9.5 y average age) with traumatic spinal cord injury to identify their suitability to enter support institutions for handicapped persons. The energy intake of all the subjects fell below the estimated energy requirement (EER), but the protein intake of most subjects met the estimated average requirement (EAR). The intake of some micronutrients fell below the EAR figure, did not meet the adequate intake (AI) level, or did not meet the tentative dietary goal for preventing life-style related diseases (DG). Although all subjects had a low body mass index and high body fat percentage, none were determined to be malnourished. There is some doubt whether DRIs can be used in the treatment of a person with motor disability, and it is necessary to accumulate further knowledge by investigation and research to clarify this for the future.
