ABSTRAK Pendahuluan: Necrotizing ulcerative stomatitis (NUS) merupakan penyakit inflamasi pada rongga mulut, ditandai oleh ulser nekrosis akut, berkembang cepat serta menyebabkan kerusakan masif pada ...jaringan. NUS dapat berkembang dari kondisi necrotizing ulcerative gingivitis (NUG) dan necrotizing ulcerative periodontitis (NUP). NUS dapat disertai infeksi lain, salah satunya infeksi herpes simplex tipe 1 (HSV-1). Infeksi HSV-1 dan NUS pada anak dengan anemia aplastik jarang ditemukan. Anemia aplastik merupakan kelainan hematologi, ditandai dengan hipoplastik/aplasia sumsum tulang dan pansitopenia, sehingga menurunkan resistensi host terhadap infeksi. Gingivostomatitis herpetik primer memudahkan perkembangan bakteri komensal seperti bakteri Fusiform, Spirochetes dan Pseudomonas aeruginosa menjadi patogen pada kondisi imunosupresif sehingga meningkatkan progresivitas penyakit periodontal menjadi NUS. Tujuan laporan kasus ini adalah menjelaskan tatalaksana infeksi HSV-1 dan NUS pada anak dengan anemia aplastik. Laporan kasus: Seorang anak perempuan usia 3 tahun dengan diagnosis anemia aplastik dan febrile neutropenia, dirujuk oleh divisi Hematologi-onkologi anak Rumah Sakit Dr. Hasan sadikin (RSHS) Bandung dengan keluhan pembengkakan pada bibir dan stomatitis. Hasil pemeriksaan darah menunjukan adanya pansitopenia dan pemeriksaan anti HSV-1 IgG reaktif. Diagnosis terkait kondisi bibir dan mulut pasien adalah Infeksi HSV-1 dan NUS. Tatalaksana yang dilakukan adalah debridement dengan Hidrogen peroksida 1,5% (H2O2 1,5%), kontrol plak dengan chlorhexidine digluconate 0,12% dan multivitamin. Pembengkakan mulai membaik setelah 3 hari debridement serta memberikan hasil yang signifikan setelah 1 bulan perawatan. Simpulan: Tatalaksana berupa peningkatan oral hygiene dan terapi antiseptik dapat memperbaiki kondisi infeksi HSV-1 dan NUS, khususnya pada pasien dengan kondisi imunosupresif seperti anemia aplastik. Kata kunci: febrile neutropenia; stomatitis; herpes simpleks; anemia; spirochetes ABSTRACT Introduction: Necrotising ulcerative stomatitis (NUS) is an inflammatory disease of the oral cavity, characterised by acute necrotising ulcers, proliferating, and causing massive tissue damage. NUS can be developed from necrotising ulcerative gingivitis (NUG) and necrotising ulcerative periodontitis (NUP). NUS can be accompanied by other infections, such as herpes simplex type 1 (HSV-1) infection. However, HSV-1 and NUS infections in children with aplastic anaemia are rare. Aplastic anemia is a haematological disorder characterised by hypoplastic/bone marrow aplasia and pancytopenia, reducing the host's resistance to infection. Primary herpetic gingivostomatitis facilitates the development of commensal bacteria such as Fusiform bacteria, Spirochetes and Pseudomonas aeruginosa to become pathogens under immunosuppressive conditions, thereby increasing the progression of periodontal disease to NUS. This case report aims to describe the management of HSV-1 and NUS infections in children with aplastic anemia. Case report: A 3-year-old female child diagnosed with aplastic anaemia and febrile neutropenia was referred by the paediatric haematology-oncology division of Dr Hasan Sadikin Hospital (RSHS) Bandung with complaints of swelling of the lips and stomatitis. Blood test results showed pancytopenia and reactive anti-HSV-1 IgG examination. The diagnosis related to the patient's lips and mouth condition was HSV-1 and NUS infection. The treatment is debridement with 1.5% hydrogen peroxide (H2O2 1.5%), plaque control with 0.12% chlorhexidine digluconate and multivitamins. After three days of debridement, the swelling started to improve and gave significant results after one month of treatment. Conclusion: Management of improving oral hygiene and antiseptic therapy can improve the condition of HSV-1 and NUS infections, especially in patients with immunosuppressive conditions such as aplastic anemia. Ensefalitis herpes simplex (EHS) seringkali berakibat fatal. Virus herpes simpleks (VHS)dapat menyerang semua umur tanpa perbedaan jenis kelamin. Angka kejadian EHSadalah 1:(250.000–500.000) ...populasi/tahun. Pada fase prodromal dapat ditemukanmalaise dan disertai demam selama 1-7 hari. Pasien mengalami penurunan kesadarandan kejang, dapat berupa kejang umum. Gejala neurologi umumnya ditemukanhemiparesis, sedangkan pemeriksaan darah tepi rutin pada EHS tidak efektif. EEG sangatmembantu bila ditemukan perlambatan fokal di daerah temporal atau fronto temporal.Titer antibodi terhadap VHS dapat diperiksa dari serum dan cairan serebrospinalis.Pengobatan VHS berupa terapi simtomatis dan suportif ditambah antivirus spesifik(acyclovir). Prognosis pasien EHS yang tidak diobati sangat buruk, sedangkan padapengobatan dini dengan acyclovir akan menurunkan mortalitas menjadi 28%.
Purpose: The aim of this study is to compare the effects of acyclovir and penciclovir creams in the treatment of recurrent herpes labialis.
Materials and Methods: Seventy patients with pre-diagnosed ...recurrent herpes labialis lesions were enrolled in this study. Diagnosis was confirmed with HSV-1 IgM antibody test. Patients were separated under 2 treatment groups as acyclovir and penciclovir cream. Both agents were applied until complete healing of the lesions was seen. Burning, itching and bleeding scores; time of erythema, vesicles, ulceration, crusting and crust loss of the lesions were recorded. Pain values of the patients were recorded using a visual analog scale (VAS) and DASS-21 scale was used to measure emotional state related to depression, anxiety and stress.
Results: Pain level in the acyclovir group was significantly lower than the penciclovir group in the 3rd day, though with no difference in the following days. DASS-21 scores revealed signs of depression on 62.8% of the patients, anxiety on 34.2% of the patients, and stress on 41.4% of the patients in moderate or higher levels. While the pain level was higher in patients with high anxiety severity, it was observed that the pain level was low in patients with high stress intensity.
Conclusion: Although it is not possible to cure recurrent herpes labialis completely, any methods and drugs that shorten the healing time of the lesion and reduce pain and discomfort gains importance. Our study showed that acyclovir is superior in reducing pain.
Giriş: Bu çalışmanın amacı, rekürrent herpes labialisin topikal tedavisinde asiklovir ve pensiklovir etken madde içeren kremlerin etkilerini karşılaştırmaktır.
Gereç ve Yöntem: Çalışmamıza rekürrent herpes labialis tanı konmuş lezyonu olan 70 hasta dahil edildi. Teşhis HSV-1 IgM antikor testi ile doğrulandı. Hastalar asiklovir ve pensiklovir etken maddesi içeren kremlerin kullanıldığı iki tedavi grubuna ayrıldı. Her iki ajan lezyonlarda tam iyileşme görülene kadar uygulandı. Yanma, kaşıntı ve kanama skorları; lezyonların kızarıklık, vezikül, ülserasyon, kabuklanma ve kabuk kaybı zamanları kaydedildi. Hastaların ağrı değerleri görsel analog skala (VAS) ile kaydedildi ve depresyon, anksiyete ve stres ile ilgili duygusal durumu ölçmek için DASS-21 skalası kullanıldı.
Bulgular: Asiklovir grubundaki ağrı düzeyi, sadece üçüncü günde pensiklovir grubuna göre anlamlı derecede düşüktü. DASS-21 skorları, hastaların %62,8'inde depresyon, %34,2'sinde anksiyete ve %41,4'ünde orta ve yüksek düzeyde stres belirtisi gösterdi. Anksiyete şiddeti yüksek hastalarda ağrı düzeyleri yüksek iken, stres yoğunluğu yüksek hastalarda ağrı düzeyinin düşük olduğu görüldü.
Sonuç: Rekürrent herpes labialisin tamamen tedavisi mümkün olmamakla birlikte lezyonun iyileşme süresini kısaltan, ağrı ve rahatsızlığı azaltan her türlü yöntem ve ilaç önem kazanmaktadır. Bu bağlamda çalışmamız, asiklovir etken madde içeren ajanın, ağrıyı azaltmada pensiklovir etken maddesi içeren ajana göre daha üstün olduğunu göstermiştir.

While the urea or lactic acid containing moisturizers, topical steroids, and topical retinoids are used in mild cases, systemic retinoids are preferred in severe cases. Kaposi varicelliform eruption caused by herpes simplex virus is rare but a well-defined complication. Clinical findings of this viral infection, which are usually uncommon, may delay diagnosis and treatment. Here, we report the case of a patient with Darier’s disease treated with corticosteroids who later developed fatal herpes simplex virus infection despite the aciclovir therapy.Key Words: Darier’s Disease; Herpes Simplex Infection; Fatal Herpes Simplex Infection. Darier Hastalığında Fatal Herpes Simpleks Virüs Enfeksiyonu Özet Darier Hastalığı, otozomal dominant kalıtımla geçen, klinik olarak gövdede ve özellikle seboreik alanlarda lokalize, küçük, hiperkeratotik papüllerle karakterize bir hastalıktır. Hastalıkta intertriginöz alanlarda vejetasyon ve infeksiyonlar sıktır. Tedavisi genellikle tatmin edici değildir. Hafif olgularda üre veya laktik asit içeren nemlendiriciler, topikal steroidler, topikal retinoidler kullanılırken, şiddetli olgularda sistemik retinoidler tercih edilir. Herpes simpleks virüsünün neden olduğu Kaposinin variselliform erüpsiyonu hastalığın nadir görülen fakat iyi tanımlanmış bir komplikasyonudur. Bu viral enfeksiyonun alışılmışın dışındaki klinik bulguları, sıklıkla tanıda ve tedavinin verilmesinde gecikmelere neden olabilir. Herpes simplex hepatitis is a rare cause of acute hepatitis in immunocompetent patients. Herpes hepatitis presents with nonspecific symptoms like fever, loss of appetite, vomiting, nausea and ...abdominal pain. Elevated transaminase values with leukopenia, relatively low bilirubin level and mucosal herpetic lesions often provide clues to the diagnosis. We describe an immunocompetent woman with herpes simplex hepatitis presenting with multiple hypodense lesions in the liver and mucocutaneous herpetic lesions.

virusom (HSV). Genitalne infekcije uzrokovane HPV-om sve su
više predmet istraživanja s ...obzirom na njihovu visoku
učestalost unutar skupine spolno prenosivih infekcija (STIs,
engleski sexually transmitted infections), sklonost recidivima,
dugotrajno liječenje i povezanosti s pojavom zloćudnih bolesti.
HPV-genitalne infekcije najčešće se pojavljuju u mladoj, generativno
sposobnoj populaciji te je stoga njihovo uspješno
praćenje i liječenje obveza svakog društva koje teži napretku.
Do danas se utvrdilo više od 100 tipova HPV-a. Genitalne
infekcije uzrokovane HPV-om klinički se najčešće manifestiraju
kao sljedeći entiteti: condylomata acuminata (šiljasti
kondilomi), condylomata plana (ravni kondilomi), gigantski
kondilom Buschke-Löwenstein i papulosis bowenoides. Danas
je neposredna detekcija HPV DNA u uzorku tkiva ili obrisku
metoda izbora u dijagnostici HPV-genitalnih infekcija. Za sada
ne postoji specifično protuvirusno liječenje HPV-genitalnih
infekcija, stoga izbor liječenja ovisi o općem stanju i dobi
bolesnika, o obliku, veličini i lokalizaciji promjena, kao i o
iskustvu terapeuta. Što se genitalnog herpesa tiče, njegovi
česti i nepredvidivi recidivi, pojava ulceracija, visoka kontagioznost
u aktivnoj fazi bolesti, kao i latentna faza infekcije
terapijski su probem i liječniku i bolesniku. Vrlo strogo
gledano, s obzirom na to da HSV u latentnoj fazi perzistira u
ganglijima živčanog sustava, još ne postoji lijek kojim bi se
problem genitalnog herpesa definitivno riješio. Velik je
napredak postignut pojavom specifičnih protuvirusnih lijekova
za liječenje aktivne faze herpetične infekcije - aciklovira,
valaciklovira i famciklovira. Postoji već velik broj i kliničkih
studija rezultati kojih pokazuju visoku učinkovitost spomenutih
lijekova u kratkotrajnom epizodnom, kao i prolongiranom,
supresivnom liječenju genitalnog herpesa.

Amaç: İdiopatik fasyal palsi (Bell palsisi) nadir ve nedeni bilinmeyen bir bozukluktur. Gebelik ve puerperiumda görülme insidansı daha yüksek olarak belirtilmektedir. Obstetrisyenler gebelikte böyle ...bir olgu ile karşılaşıp tedavi planını yapmak zorunda kalabileceği için, son zamanlarda karşılaştığımız bir olgu ile bu konuyu tekrar gözden geçirmek istedik.

Materyel ve Metod: Gebeliğin 30. Haftasında idiopatik fasyal palsi saptanan primigravid bir gebe
Bulgular: İdiopatik fasyal palsi tanısı konulunca derhal fizyoterapi ve yüksek doz B vitamini tedavisi uygulandı. Sağlıklı bir erkek bebek doğurduktan sonra, tam iyileşme saptandı.
Sonuç: Gebelikte idiopatik fasyal palsinin daha sık görülmesinin nedeni, gebelikteki immun değişikliklere bağlı olarak, lokal ödeme yol açarak nöral bası yaratan Herpes simplex aktivasyonu yoluyla açıklanabilir. Prognoz, özellikle hasta genç ise oldukça iyidir. Erken fizyoterapi iyi sonuç için son derece önemlidir.
Objective: Although idiopathic facial palsy or Bell's palsy is an infrequent enigmatic disorder it has higher incidence in pregnancy and puerperium. Since obstetricians may be obligated to manage the Bell's palsy in relation to pregnancy, we wish to review this subject by a recently encountered case.

Material and Method: A 23 year-old primigravid woman presented with Bell's palsy at 30th week of pregnancy.
Results: She was treated with physiotherapy and high dose B-group vitamins as soon as the diagnosis of idipathic facial palsy was established. After she has delivered a healty baby boy , complete reversal was confirmed.
Conclusion: Higher incidence in relation to pregnancy may be explained with Herpes simplex activation due to immunological alteration in pregnancy, causing local edema, and thus, resulting in neural compression. The prognosis is good, especially when the patient is young. The earlier physiotherapy is the key for better outcome.
Erythema multiforme (EM) je heterogeni sindrom klinički obilježen čestim opetovanim epizodama simetrično raspoređenih numularnih kožnih promjena koje koncentrično prelaze u promjenu izgleda. Spektar ...EM uključuje izbijanje znakovitih kožnih promjena prvenstveno smještenih na ekstenzornim stranama ekstremiteta (EM minor) ili teške sistemske bolesti s erozijama na nekoliko sluznica zajedno s promjenama kože (Stevens-Johnsonov sindrom, SJS) ili konfluirajuće areale lamelozne deskvamacije (toksična epidermalna nekroliza, TEN). Opseg bolesti je prilično varijabilan: od tipičnih irisu sličnih formacija na gornjim ili donjim ekstremitetima bez promjena na sluznici usne šupljine do opsežne kliničke slike s tipičnim i atipičnim iris formacijama na gornjim ili donjim ekstremitetima uz opsežno zahvaćanje sluznice usne šupljine i teškim općim simptomima (SJS). EM je znatno češći u mladih ljudi i traje nekoliko tjedana, ovisno o početnoj zahvaćenosti kože. Smatra se da je EM poseban oblik kožne reakcije na različite uzročne čimbenike, kao što su opetovana infekcija herpes simpleks virusom (HSV), mikoplazma ili bakterijska infekcija, rijetko lijekovi, x-zrake ili sarkoidoza. U mnogim slučajevima ovi agensi se ne mogu dokazati. Terapija uključuje simptomatske mjere za ublažavanje svrbeža i boli, često profilaktički peroralno aciklovir za ponavljajuće EM uzrokovane virusom HSV. U potencijalno teškim slučajevima sistemski kortikosteroidi mogu se prikladno upotrijebiti u ranoj fazi bolesti radi ublažavanja promjena kože i sluznica, ali potporna njega i suzbijanje sekundarnih infekcija i drugih komplikacija su ključni elementi u liječenju EM.
