Licorice root is a widely consumed drink in warm seasons, especially in the East of Turkey. This drink is consumed plentifully especially in the month of Ramadan. We hereby presented 3 cases with ...muscle weakness, hypokalemia, rhabdomyolysis and hypertension developed secondary to excessive consumption of licorice root, which resolved with potassium replacement.
Meyan kökü, özellikle Türkiye'nin doğusunda sıcak mevsimlerde yaygın tüketilen bir içecektir. Bu içecek özellikle ramazan aylarında bol miktarda tüketilmektedir. Bu yazıda, aşırı meyan kökü tüketimi sınrası kas güçsüzlüğü, hipokalemi, rabdomyoliz ve hipertansiyon gelişen ve potasyum replasmanı ile klinikleri düzelen 3 olgu sunduk.
Amaç: Mevsimsel tarım işçilerinde çömelerek uzun süreli çalışmaya bağlı olarak gelişmiş peroneal sinir felci hastalarını, klinik tablo ve elektrofizyolojik çalışmalarla değerlendirilerek takip ve ...tedavi sonuç¬larını sunuldu ve bu sorunun tedavisi ve önlenmesine yönelik bir yaklaşım yolu belirlemeye çalışıldı.Çalışma planı: Polikliniğimize başvurmuş peroneal sinir felci hastalarından çömelerek çalışma hika¬yesi olan ve mevsimsel tarım işçisi 16 hastanın 18 ayağı retrospektif olarak değerlendirildi. Hastaların klinik ve nörolojik muayene değerlendirilmeleri not edildi ve kilo kaybı hikâyesi sorgulandı. Kilo ve boy değerleri elde edilerek vücut kitle indeksleri hesaplandı. Elektrofizyolojik testleri ile tanı doğrula¬narak lezyon yeri belirlendi. Tanı sonrası tüm hastalara medikal tedavi verildi ve ortez kullanması ve istirahat önerildi. Takipler sırasında hastalara ayak bileği hareketleri önerildi ve şikâyetleri geçen has¬taların ortez kullanımı sonlandırıldı. Takipler motor şikâyetlerin tamamen düzelmesine kadar yapıldı.Bulgular: Tüm hastaların klinik ve nörofizyolojik değerlendirme sonucu izole peroneal sinir felci ile uyumlu idi. Çalışma kapsamına alınan 16 hastanın (9 kadın, 7 erkek) ortalama yaşı 23.6 idi. Yedi has¬tanın sol, yedi hastanın sağ ve iki hastanın her iki ekstremitede peroneal sinir felci geliştiği görüldü. Şikâyetlerin başlangıcının bir ile altı hafta arasında idi (ort. 3.1 hafta) ve günlük çömelerek çalışma süresi ortalama 6.8 saat idi (6 ile 8 saat arası). Ortalama iyileşme süresinin 7.3 hafta olduğu görüldü. Hiçbir hasta kilolu veya obez sınıfta değildi. On iki haftalık takipte iyileşme görülmeyen iki hastaya cerrahi gevşetme uygulandı.Çıkarımlar: Peroneal sinir felcinde hastaların çalışma şekilleri ve meslekleri de göz önünde bulundu¬rulmalıdır. Çömelerek çalışmaya bağlı görülen peroneal sinir felcinde istirahat, konservatif tedavi ve yakın takip ile iyileşme beklenmelidir.
Objective: The aim of this study was to evaluate the follow-up and treatment results of peroneal nerve palsy secondary to prolonged squatting for working and to determine an approach for its treatment and prevention.Methods: The study retrospectively evaluated 16 patients (7 males, 9 females; mean age: 23.6 years) diagnosed with peroneal nerve palsy due to squatting for work. Clinical and neurological evaluations were performed and weight and height were measured. Lesion site was determined using electrophysiological testing. After diagnosis, medical and orthotic treatment was initiated and rest was advised.Patients were followed until motor symptoms were resolved.Results: Clinical and neurophysiological evaluations were consistent with isolated peroneal nerve palsy. The left side was affected in seven patients, the right side in seven and both sides in two. Average onset of the symptoms was 3.3 (range: 1 to 6) weeks and average daily squatting period was 6.8 (range:6 to 8) hours. Average healing time was 7.4 (range: 3 to 16) weeks. None of the patients was obese or overweight. All patients healed with conservative treatment and no surgical treatment was necessary.Conclusion: Working conditions and duties should be considered in the evaluation of peroneal nerve palsy. In peroneal nerve palsy secondary to squatting, healing should be expected with conservative treatment, resting and close follow-up.
Licorice is the root of Glycyrrhiza glabra, which has a herbal ingredient, glycyrrhizic acid. Excessive intake of licorice may cause a hypermineralocorticoidism-like syndrome characterized by sodium ...and water retention, hypokalemia, hypertension, metabolic alkalosis, low-renin activity, and hypoaldosteronism. In this paper, an 34 years old man who admitted to the emergency department with respiratory failure and marked muscle weakness of all extremities that progressed to paralysis after excessive intake of licorice syrup was presented. It was aimed to draw attention to the necessity of questioning whether there is excessive intake of licorice or not in patients who admitted to emergency department with paralysis and dyspnea. Plasma potassium concentration of the patient was 1.4 mmol/L. The patient's respiratory distress and loss of muscle strength recovered completely after potassium replacement.
Meyan kökü glycyrrhizic acid içeren Glycyrrhiza glabra"nın köküdür. Aşırı meyan kökü alımı sodyum ve su retansiyonu, hipokalemi, hipertansiyon, metabolik alkaloz, düşük renin aktivitesi ve hipoaldosteronizm ile karekterize hipermineralokortikoidizm benzeri sendroma yol açabilir. Bu yazıda aşırı meyan kökü şerbeti içimi sonrası acil servise solunum sıkıntısı, kol ve bacaklarında güçsüzlük nedeniyle getirilen hipopotasemik paralizili 34 yaşındaki erkek hasta sunuldu. Acil Servise paralizi ve solunum sıkıntısıyla gelen hastalarda aşırı meyan kökü alımı olup olmadığının sorgulanması gerekliliğine dikkat çekilmesi amaçlandı. Kan potasyum düzeyi 1.4 mmol/L olan hastanın potasyum replasmanı sonrası solunum sıkıntısı ve kas kuvvet kaybı tamamen düzeldi.
Paralizia cerebrală reprezintă un grup de tulburări de mişcare, postură, tonus, funcţie motorie, persistente (dar nu neapărat neschimbate), neprogresive, cu debut precoce, datorate unor afecţiuni ...neprogresive, survenind pe un creier imatur sau în dezvoltare (prenatal, perinatal, postnatal în primii 3-4 ani de viaţă). (1) Se asociază în mod variabil cu: tulburări cognitive, epilepsie, defi cite senzoriale, tulburări de comportament. Etiologia paraliziei cerebrale este uneori difi cil de identifi cat, iar neuroimagistica joacă un rol important în acest sens. Scopul lucrării este de a analiza rezultatele investigaţiilor imagistice în relaţie cu clinica. Material şi metodă: este un studiu retrospectiv al pacienţilor internaţi în Clinica de Neurologie pediatrică a Spitalului Al. Obregia în 2010 cu diagnosticul de paralizie cerebrală şi care au avut investigaţii imagistice. Rezultate şi concluzii: au fost analizaţi 379 copii. Majoritatea copiilor (> 87%) cu paralizii cerebrale au avut anomalii neuroradiologice. Pe primul loc ca frecvenţă s-au situat leziunile de substanţă albă, iar pe locul doi leziunile de substanţă albă şi cenuşie. Leziunile de substanţă albă periventriculară s-au corelat cu dipareza spastică. Leziunile de substanţă albă şi cenuşie s-au corelat cu hemipareza spastică, iar cele de substanţă cenuşie cu formele dischinetice. Malformaţiile au fost semnifi cativ asociate formelor ataxice şi hemiparezelor spastice. Imagistica a fost mai frecvent normală în diparezele spastice. Datele sunt concordante cu cele din literatură. Investigaţiile imagistice au rolul de a contribui la înţelegerea etiologiei şi patogeniei paraliziei cerebrale, formularea unui prognostic, iar pe viitor la găsirea profi laxiei.
Exprimarea clinică a tulburărilor de somn este foarte variată, distingându-se tulburări legate de adormire şi manifestări patologice care apar în timpul somnului. Scopul studiului a fost de a ...determina prezenţa tulburărilor de somn la 129 de pacienţi de vârstă pediatrică cu paralizie cerebrală (PC). Pentru diagnosticul tulburărilor de somn a fost completat de către părinţi Pediatric Sleep Questionnaire. La 61 (47,28%) dintre pacienţii cu PC urmăriţi s-au depistat probleme de somn. Cele mai frecvente tulburări de somn au fost reprezentate de difi cultăţi în iniţierea somnului (50,81%), treziri nocturne frecvente (44,26%) şi insomnie (49,18%). Tulburările de somn pot fi prezente la orice vârstă la copiii cu PC. Cele mai frecvente manifestări sunt manifestările legate de adormire şi parasomniile.

Dokuz yaşında erkek, sol ayağında ani gelişen kuvvet azlığı ve yürüme güçlüğü ile değerlendirildi. Fizik muayenesinde özellik olmayan olgunun, nörolojik muayenesinde sol alt ekstremite distalinde dorsifleksiyon zaafı vardı. Elektromiyografi incelemesi, basınca duyarlı herediter nöropati'yi düşündürdü. DNA analizi, basınca duyarlı herediter nöropati'ye sebep olan, 17p 11.2-12 delesyonunu gösterdi. Olgu, çocuklarda nadir görülmekle birlikte basınca duyarlı herediter nöropati'nin, akut gelişen paralizilerde akılda tutulmasının vurgulanması amacıyla sunulmuştur.
