Depuis une décennie, le péritoine est considéré comme un organe à part entière. Une meilleure compréhension de l’anatomie, du rôle et de la physiopathologie du péritoine a permis de faire des avancées sur les plans diagnostique et thérapeutique des métastases péritonéales d’origine colorectale (MPCCR). Le seul traitement curatif des MPCCR repose sur la cytoréduction complète associée à une chimiothérapie systémique ± chimiothérapie hyperthermique intrapéritonéale après discussion en réunion de concertation pluridisciplinaire spécialisée en maladies péritonéales (réseau RENAPE). Nous allons aborder, dans cet article, les points importants permettant de diagnostiquer et de prendre en charge les MPCCR à visée curative. On closer inspection, the peritoneum is definitively an organ with a very complex anatomy. Understanding its anatomy/physiology and the mechanism of peritoneal dissemination from the primary tumor plays a key role in the prevention, early detection and management of colorectal peritoneal metastasis (cPM). Cytoreductive surgery combined with preoperative chemotherapy and/or hyperthermic intraperitoneal chemotherapy with Mitomycin became a curative option for cPM in selected patients. In this article, we will discuss diagnosis and management of cPM in curative context.
