    Aim. During ischemia-reperfusion injury cellular calcium regulation in cardiomyocites is compromised, leading to mitochondrial calcium overload andpossiblly cell death. The myocardial contractility ... is diminished, and because of the inhomogeneity of cell viability reperfusion arrhythmias arise. Acute renal failure (ARF) is manifested by disturbed myocardial calcium regulation, leading to enhanced mitochondrial calcium concentration. In the present experiments we studied involvement and consequences of disturbed myocardial calcium regulation on ischemia-reperfusion injury in guinea pig hearts with ARF. Materials and methods. Guinea pigs were injected with gentamicine sulphate (80 mg/kg/d) for 8 days; control animals received the same volume of saline. Isolated Langendorffs hearts underwent 50 min of zero-flow ischemia and 60 min of reperfusion. We evaluated the influences of calcium antagonists mibefradil, acting at the T-type calcium channels and partly at the L-type calcium channels, and of verapamil, actihg at the L-type calcium channels only, upon ischemic-reperfusion injury. During the time of induction of ARF mibefradil or verapamil (2 mg/kg/d) were injected 8 days (subchronically) or were added into the heart perfusion solution (0,1 or 1 p.M) (either during reperfusion only or before ischemia and during reperfusion). We registered left ventricular developed pressure (LVDP), heart rate and arrythmias, coronary flow and LDH release rate during myocardial reperfusion. Results. During reperfusion the incidence of unrecoverable ventricular fibrillation in ARF hearts vs. control hearts led to myocardial contractility failure. After adding mibefradil into perfusate ventricular fibrillation was either much shorter in duration or was completelly abolished.LVDP was diminished in the reperfused ARF hearts when compared to control reperfused hearts.
