  • Komorbidnost kroničnih somatskih bolezni in depresivnih znakov v ambulanti splošne medicine = Comorbidity of chronic somatic diseases and depressive signs in general practice

    Clinical practice suggests that many patients with chronic somatic disease whoattend general practitioners show symptoms of depression and anxiety. They can be presented as separate entities, in ... mixed clinical picture of symptoms, or coexist in the sense of comorbidity. A comparative study was made to research this problem. We set up two goals of the study. First was to estimateand compare the degree of depression and anxiety in the two groups of patients with chronic somatic disease. The second goal was evaluation of comorbidity and cosimptomatology of depression and anxiety. Groups of 40 orthopedic and 39 diabetic patients who attended general setting in a 3-month period were compared by using Hamilton depression and Hamitton anxiety rating questionnaire to get the scores of depression and anxiety. In orthopedic group20% of patients had mild depression and 17.5% filled the criteria for moderate to severe depression. In diabetic group 12.8% of patients had mild and none had moderate to severe depression. Orthopedic patients showed 35% of mild anxiety and 32.5% of major anxiety. In diabetic group 30.8% of patients had minor and 7.7% major anxiety. The comparison between the groups showed statistically significantly higher depression and anxiety scores in orthopedicgroup (p<0.05). Quality of life was tested with Who Ten Well Being questionnaire to help the patients describe their own subjective experience ofillness. Orthopedic patients showed higher scores, having more impaired quality of life. Another type of comorbidity was present among depression and anxiety. Among 27 orthopedic patients with depressive signs and/or anxiety comorbidity of these two disorders was found in 26%, cosymptomatology in 29% and mild anxiety in 45%.
