Uporaba NT-proBNP pri bolnikih s srčnim popuščanjem = Use of NT-proBNP in patients with heart failure

BNP (brain natriuretic peptide, B-type natriuretic peptide) has diuretic, natriuretic and vasodilating effects. The precursor of a biologically active form is produced in cardiac myocytes of both ...ventricles. NT-proBNP and active molecule BNP are released into blood. In healthv subjects their concentration in blood is low; however, it inereases markedly in congestive heart failure asa result of increased myocardial stretch. Serum concentrations of BNP and NT-proBNY are correlated to the degree of myocardial damage and to NYHA class.BNP and NT-proBNP have very good negative predictive value for ruling out acute heart failure as a cause of acute dyspnea. Increased serum concentrations of BNP and NT-proBNP contribute to diagnosing congestive heart failure and left ventricle dysfunction. They also carry prognostic informationin congestive heart failure. After successful treatment of congestive heart failure serum BNP and NT-proBNP decrease. Currentlv, BNP and NT-proBNP are not yet routinely used in everyday clinical practice for treatment monitoring in patients with congestive heart failure.
