  • Test pomnoževanja nukleinskih kislin iz bronhialnega izpirka v diagnostiki pljučne tuberkuloze = The value of nucleic acid amplification test from bronchoaspirate in the diagnosis of pulmonary tuberculosis

    Background. In the study the efficacy of amplification test (AT) in bronchial aspirate (BAS) for the diagnosis of active pulmonary tuberculosis was evaluated. Methods. Between January 1995 and ... December 1996, 188 BAS from 185 patients, suspected of having active tuberculosis, were examined microscopically for acid fast bacilli (AFB), AT and culture. The sputum smearsbefore fiberoptic bronchoscopy (FOB) were negative or sputum couldn't beobtained. Results. Among 185 patients (72 females and 113 males, mean age = 57.5+-15.8) who were referred for FOB, active tuberculosis was established in 31 patients (17%). AT from BAS was positive in 23 patients (sensitivity in comparison with final diagnosis is 64.5%), three patients were false positive (specificy = 98.1%). The positive predicted value of the AT from BAS was 87%. The negative predicted value of the AT from BAS was 93.2%. Conclusions. These findings suggest that the AT inbronchial aspirate is a useful and in comparison with bronchoscopic lung biopsy noninvasive method for rapid diagnosis of "smear negative" pulmonary tuberculosis. It is also important in exluding TB among patients with symptoms of other pulmonary diseases.
