    The survival and quality of survival of patients with advanced carcinoma of the pharyngo-laryngeal region were analyzed in 41 patients following laryngo-pharyngectomy and reconstruction with jejunal ... free flap and in 31 irradiated-only patients of comparable cancer localisation and stage. Surgery gave superior oncologic results but poorer quality of life compared to radiotherapy alone. The survival was poor in both groups. Therefore, the best possible quality of survival is the most important goal to achieve. A prospective randomised study to compare chemo-radiotherapy with surgery and postoperative irradiation would be necessary to make a step forward in the therapeutic approach.
