Ischemic and congestive myelopathies form two broad groups of rare neurological conditions, each caused by several individual etiologies. The more obvious manifestation of arteriovenous malformations ... (AVM) in the spinal canal is an acute neurological syndrome due to hemodynamic or hemorrhagic problems. Congestive myelopathy is the main clinical manifestation of spinal dural arteriovenous fistula, the most common AVM of the spinal canal. Clinical symptoms and findings include motor, spinothalamic, and lemniscal sensory deficits depending on the spinal level and the pattern of ischemic myelopathy. Anterior spinothalamic tract lesions in the affected segments cause dissociated sensory loss in body parts below the lesion. It has been shown to be due to venous hypertension, most often resulting from a dural arteriovenous fistula tiny connection between a radicular artery and vein that impedes venous drainage of the spinal cord. Gait difficulties and muscle strength respond better than micturition, pain, and muscle spasms.
