  • Predoperativna ocena kronične mitralne regurgitacije = Preoperative assessment of cronic mitral regurgitation

    In the management of patients with chronic mitral regurgitation and selection of the most appropriate time for surgical interention in these patients, several facts discussed in the article should be ... considered. Evaluation on thebasis of clinical and echo-Doppler parameters is emphasised. Mitral regurgitation leads to an eccentric hypertrophy and progressive decline in left ventricle pump function secondary to contractile dysfunction. Patients with a normal left ventricle pump function and a reversible contractile dysfunction have a favourable long-term postoperative death becouse of the persistent left ventricle dysfunction, but the survival rate is improved with surgical therapy in comparison with medical therapy. Mitral valve reconstruction preserves the left ventricle function and reduces mortality andcomorbid complications in comparison with conventional mitral valve replacement. Mitral valve reconstruction shoul be the procedure of choice in patients with reduced preoperative left ventricle function.
