  • Diagnostična vrednost renografije s kaptoprilom in ultrazvočne doplerske preiskave ledvičnih arterij v prepoznavanju renovaskularne hipertenzije : magistrska naloga

    In prospective study we have compared sensitivity and specificity of captoprilrenography (CR) and duplex doppler sonography (DDS) in detecting haemodynamically significant renal artery stenosis (>= ... 60 %) and positive predictive values of both tests for cure or improvement of hypertension following percutaneous transluminal renal angioplasty (PTRA). Between January 1993 and June 1996 we have followed 45 patients with moderate or high index ofclinical suspicion for renovascular hypertension (RVHT). All patients underwent both CR and DDS. If the result of at least one of both methods was positive, renal angiography was performed. Patients with agiographically proven >= 60 renal artery stenosis were treated with PTRA, unless the procedure was contraindicated. Renal angiography revealed renovascular diseasein 80% of patients. The high prevalence of renovascular disease in our group of patients confirms the accuracy of clinical criteria we have followed in selection of the patients. The results of CR and DDS were compared to the results of renal angiography in 74 renal arteries. Sensitivity of CR in detecting >=60 % renal artery stenosis was 82,1 and specificity 87 %. Sensitivity and specificity of DDS were 82,1 % and 89,1 %, respectively. The predictive value of CR and DDS for improvement of blood pressure following technically successful PTRA, was asessed in 18 patients. Immediately after thePTRA and after mean follow up of 10,1 +- 1 months, 83,3 % of patients were cured or improved. In 16,7 % of patients treatment was clinically unsuccessful. Positive predictive values of CR and DDS for cure or improvementof hypertension after PTRA were 86,7 % and 83,3 %, respectively. CRand DDS are equivalent tests for the detection of haemodynamically significant renal artery stenosis in selected patients with moderate or high index of clinical suspicion of RVHT.
