    Asthma is a chronic inflammatory disease. Symptoms of the disease can be almost completely prevented by anti-inflammatory treatment. Many studies showed that viral infections are the major trigger of ... asthma exacerbation. In 10 year old children at least 80% of exacerbations were associated with viral infections, most commonly with rhinoviruses (65%). Rhinoviruses enter epithelial cells using the adhesion molecule ICAM-1 as a receptor. Asthma patients are particularly susceptible to rhinovirus infection, as ICAM-1 molecule expression is upregulated in asthma. The consequence of infection is destruction of the epithelium, release of proinflammatory cytokines and additional upregulation of the adhesion molecules which result in enhanced inflammation, enhanced bronchial hyperresponsiveness and finally asthma exacerbation. In the treatment of asthma exacerbation antibiotics are used only, when there are convincing signs of bacterial infection. Beside inhaled bronchodilators, anti-inflammatory treatment with glucocorticoides is the mainstay of the therapy.
