  • Contemporary cerebrospinal fluid analysis in multiple sclerosis

    Cerebrospinal fluid (CSF) examination has an added value in the diagnosis of multiple sclerosis (MS) because it reveals the inflammatory nature of the disease and points to alternative diagnoses in ... case of atypical CSF findings. Although not specific for MS, the presence of oligoclonal bands (OB) in CSF increases the diagnostic certainty and may substitute for dissemination in time according to the 2017 revisions of McDonald criteria, intended to facilitate earlier MS diagnosis. In addition to OB detection, novel assays for quantification of CSF immunoglobulin free light chains (FLC) have already entered the clinical practice. Intrathecal kappa FLC synthesis (KFLC index) displays similar diagnostic sensitivity as OB but with an obvious advantage of easier determination and objective interpretation. CSF at the time of diagnosis also offers a great opportunity for exploration of novel biomarkers with differential diagnostic or prognostic potential. One of the most promising is neurofilament light (NFL), shown to be closely associated with disease activity.
