    Background. The major issue concerning the performance of an in vitro fertilization procedure in the unstimulated cycle is determination of the mostappropriate moment for triggering ovulation. ... Various diagnostic and predictive procedures have been used for the purpose - their clinical value can be assessed by success rates of in vitro fertilization procedures. Aim. Toevaluate the effect of clinical, biochemical and biophysical parameters on the outcome of an in vitro fertilization procedure in the unstimulated cycle. Patients and methods. Retrospectively we analyzed prospectively collected dataon 399 cycles of in vitro fertilization in the unstimulated cycle of 240 patients aged less than 40 years in whom ovulation was triggered. We compared the values of various parameters, important for predicting the LH surge (serumE2 level, follicular size, endometrial thickness, day of the cycle), between those obtained in unstimulated cycles with ovulation induction and theones obtained in spontaneous ovulation cycles. Additionally, we compared the values of these parameters between successful and failed unstimulated cycles, and evaluated their effect on the successful outcome separately for high and low levels. We assessed the effect of the increase in E2 before ovulation on pregnancy rates in unstimulated cycles, and evaluated the achieved fertilization rates in terms of the day of embryo transfer and the embryo developmental stage. Results. The comparison with the spontaneous cycleshowed that in the unstimulated cycle ovulation was triggered at the timewhen the values of the observed parameters (serum E2 level, follicular size, endometrial thickness, day of the cycle) were lower one day before the LH surge and higher two days before LH surge than in the spontaneous cycle.
