    The average mortality of the first variceal bleeding is still 30-40%. Primary prevention of the first bleeding is the most important factor influencing the outcome. The aim of the pharmacological ... treatement in primary prevention is the lowering of the HPVG for more than 20% from the baseline value or below 12mmHg. The treatement of choise are nonselective beta-blokers. Nitrate and other drugs can also lower portal pressure, however because of significante side effects they are not used. Preventive band ligation is the treatement option in patients with large varices and contraindications for beta-blokers.
