    Cardiac resynchronization therapy (CRT) has been shown to improve symptoms andvarious functional parameters in patients with severe heart failure. The finding of an intraventricular conduction delay ... has been associated with clinical instability and an increased risk of death in patients with heart failure. Selection criteria for trials have included patients with NYHA class II - IV heart failure, QRS duration of > 120 ms, and ejection fraction of < 35%. Some have proposed that this intraventricular conduction delay may further impair the ability of the failing heart to eject blood and may thus enhance the severity of regurgitant flow through the mitral valve. Devices that make use of atrial-synchronized biventricular pacing to coordinate right and left ventricular contraction have been developed, and early studies have suggested that short- and long-term cardiac resynchronization can improve cardiac function and enhance functional capacity and the quality of life. Various questions about the therapy remain unanswered, including the prediction of "responders" to the therapy and therefore appropriate selection of patients.
