    A successful and save endoscopic sinus surgery of the frontal sinus depends onthe knowledge of the anatomy. The ethmoid infundibulum is the most importantand the frontal recess is the most ... complicated structure. The frontalsinus continues to present the surgeon with the most challenge, both interms of the surgical procedure and in terms of the potential for persistentand recurrent disease. The most common immediate cause of frontal sinus disease is the presence of infundibular disease displacing the uncinate process medially and the presence of agger cells' disease. We have had 440 removals of ethmoidal cells with frontotomy at the Department of otorhinolaryngology in Ljubljana in the last 17 years. The most common causes were chronic hyperplastic rhinosinusitis and mucocoeles. Most of the patients feel improvement or are without difficulties. The success of the surgery does not depend from frontotomy only but from cures and removals of other causes too.
