Introduction: Joint replacement surgery, a very frequent and successful procedure worldwide, has greatly improved the quality of life of many patients. Although infections of prosthetic joints are ... rare (1%-5%), they represent the most serious complication following prosthesis implantation. Theaim of our study was to determine the frequency of hip and knee prosthesis infection and the method to prevent therm. Methods: The study was performed with the retrograde analysis of the data of patients treated in General Hospital Celje in the year 1996 and in the period from 1998 to 2003. The microbiological data until the 1999 were obtained using the computer program Mbio, and for the period from 2000 to 2003 by using the computer program MBL. Results: In the 7-year period, there were 29 patients with infected prosthesis, 13 male, 16 female, median age 71.8. Infection rate was from 3.1%,in the year 2001 to 1.3% in the year 2003. Infection occurred in less than one month after surgery in 12 (41.4%), in the second and third month in 2(6.9), by the second year in 3 (10.5%), and after more than two years in 10 (34.5%) patients. There are no data for two patients. Polymicrobial infection was found in 7 (24.0%j cases. The most common etiological agents were, Staphylococcus aureus 13 (32.0%), Streptococcus spp. 7 (23.3%), Enterobacterieceae 7 (23.3%) and coagulase-negative staphylococci 6 (20.7%). Conclusion: There are not enough cases for statistic analysis. Nevertheless, the survey of results allows us to evaluate the quality of our work. If adequate sampling is assured, microbiological examination may substantially contribute to the recognition and therapy of infections. We recommend introducing a standardized national surveillance of prosthetic infections thatwould enable a comparison between different institutions and improvement of the procedure.
