  • Učinki nesteroidnih protivnetnih zdravil na prebavila in vloga zaviralcev protonske črpalke = Gastrointestinal side-effects of non-steroidal anti-inflammatory drugs and role of proton pump inhibitors

    Non-steroidal anti-inflammatory drugs (NSAIDs) are among most frequently prescribed drugs in modern medicine. Between 2 and 4 % of patients, taking conventional NSAIDs, wiII experience symptomatic ... ulcer or potentially life threatening ulcer complications. Approximately 100.000 hospital admittions and16.000 deaths annually are caused by NSAIDs in the USA. In Slovenia we haveapproximately 1.000 hospitalization and 40 deaths annually caused by NSAIDs. Serious complications (bleeding, perforations, stenosis) caused by synthesis inhibition of protective prostaglandins E2 and I2, local topic effect of NSAIDs and their negative effect on mucosal adaptation and healing. Three clinical factors, i. e. the presence of the previous gastrointestinal event, current Aspirin use and age over 65 years, have the greatest influence on the development of complications. The use of corticosteroid's and warfarinetreatment is also important. The role of Helicobacter pylori and recommendations for treatment are presented. Patients at higher risk of complications need protection with proton-pump inhibitors or a change of conventional NSAIDs for COX-2 inhibitors. Indications for the use of COX-2 inhibitors or proton-pump inhibitors are presented.
