Vpliv poškodbe in časa oskrbe na spremembe prepustnosti črevesne stene pri politravmatiziranih poškodovancihPolanec, Beno ; Repe, BenoBackground Because of compensatory processes in trauma shock intestinal permeability increases, so bacteries and endotoxins can enter sistemic circulation. This can be cause to later complications. ... That is why it is important to recognize and treat those patients promtly. Aim The purpose of our study was to find out how much does increased intestinal permeability influence y the treatment of polytraumatised patients. Hypothesis Our hypothesis was that intestinal permeability after severe injury is increased and in correlation with type of injury and transport time. Thus it also influences the duration of treatment and survival rate of polytraumatised patients. Methods We have prospectively studied adult polytraumatised patients, hospitalized in Irnensive care unit (ICU). We calculated shock indexat the place of injury, transport time, the severity of injury was assessed with ISS (iiyury severity score) and APACHE II (Acute Physiology and Chronic Health Evaluation) upon admission. lntestinal permeability was measured using the lactulose mannitol (L/Ivl) test on second, fourth and seventh day after injury. We have also collected the data about ihe time spentin ICU, in hospital and survival rate of those patients. Results We studied 77 polytraumatised patients. The median of their age was 37 (13,2 - 53,8) years. There were 79% male patients. Upon admission they had the median of ISS 29 (22 - 35,8). APACHE II was 13,5 (8-18). Polytraumathised patients were in average transported 54 minutes (39-75) before admitted to hospital. Average time of therapy in ICU of University Medical Centre Ljubljana was 10,5(5,25-16), in hospital 46 (26-79) days. 11 injured patients died during the treatrnern. L/M index values were 0,083 (0,015-0,352) on second day, 0,184(0,059-0,545) on fourth day and 0,263 (0,087-0,415) on seventh day.
