Early diagnosis of neonatal severe infection is ... extremely difficult because clinicaland laboratory signs are non-specific and microbiological tests are relatively slow and have low sensitivity. Consequently, antibiotics are overused in critically ill neonates. The aim of this study was to determine whether two new markers of severe infection, interleukin-8 (IL-8) and procalcitonin (PCT), are more sensitive and specific than routinely used laboratory tests: white blood cell and differential count, platelet count, serum C-reactive protein (CRP) and interleukin-6 (IL-6) concentrations. The additional aim was to find out whether sequential determination up to 48 hoursafter admission to ICU improves diagnostic value of IL-8 and PCT and whether the two markers are correlated with the virulence of the causative microorganisms of neonatal severe infection and with the neonatal illness severity. Patients and methods: Sixty-four neonates admitted to ICU who had clinical and/or laboratory signs of severe infection were included into the study. They were divided into three groups: group 1 - severe infection (n = 28; microbiologically confirmed in 10 neonates), group 2 respiratory distress without infection (n = 13) and group 3 - hypoxic injury without infection (n =22). All neonates received systemic antibiotic and the necessary supportive therapy. Blood samples obtained from central venous catethers were collected at the time of admission, at 24 hours, and at 48 hours after admission. Routine laboratory and microbiological tests were done, and serum IL-8 and PCTconcentrations were determined. Reference serum IL-8 and PCT concentrationswere obtained from cord blood of 25 healthy neonates.
