    Background and aims. Diabetes mellitus is among the most prevalent chronic diseases of the modern world. The excess morbidity and mortality of diabetic patients in comparison to non-diabetic ... population result mainly from chronic complications of diabetes. These can be divided into macro- and microangiopathic. Lower limb amputations in diabetic patients represent almostone half of all non-traumatic amputations and foot complications are among the most common causes for hospital admission of diabetic patients. The role of peripheral obliterative macroangiopathy in the pathogenesis of diabetic gangrene is well established, on the other hand the opinion on the role of microangiopathy is less equivocal. Skin nicrocirculation has been subject to extensive investigation in the last decades, however he majority ofthe published data are about microcirculation in the skin areas with ombinednutritive and thermoregulatory blood flow, after provocation with various physical and pharmacological stimuli. The data on basal blood flow in skin areas with predominantly nutritive blood flow are scarce, as well as dataon the comparison between he upper and lower extremities. Cardiovascular autonomic neuropathy is a significant risk factor for cardiac arrhythmias nd sudden cardiac death. Heart rate variability (HRV) is a strong marker of autonomic ervous system activity. Spectral analysis of HRV ia a valuable non-invasive diagnostic tool for the evaluation of short-term regulation of the cardiovascular system. Wavelet transform is a novel method for the analysis of HRV which enables better resolution of the low-frequency spectral components than the more commonly used fast Fourier transform and autoreggressive spectral model estimation. This is of great importance in particular in those diabetic patients who do not yet fulfill the standard diagnostic criteria for autonomic neuropathy. (Abstract truncated at 2000 characters)
