    Prolonged endurance exercise is potential detrimental to heart in both, trained and untrained subjects. The myocardial injury may manifest as exercise-induced cardiac fatigue or exercise-induced ... myocardial damage. Exercise-induced cardiac fatigue is a transient reduction in cardiac function,including depressed cardiac inotropic state, regional wall motion abnormalities and/or diastolic dysfunction subsequent to prolonged endurance exercise in healthy humans, while exercise-induced myocardial damage is markedby transitory elevation of cardiac markers (troponin) following prolonged endurance exercise. Both entities can occur alone, or simultaneouslywith a limited correlation between them. The most important factors inducing myocardial injury and cardiac failure appear to be duration and intensity of exercise. Many enviromental factors (temperature, humidity, altitude, hydration status, gender, age, genetics) as well as certain undefined factors may play a significant role. The short-term prognosis is excellent with normalization of cardiac troponin levels and cardiac function, while the long-term prognosis, prognosis of repetitive episodes of mycoardial injury or the impact on athletic performance are not known. Identification of persons who will develop myocardial damage or cardiac dysfunction and the treshold are not possible to date.
