    The modern insulin therapy of patients with insulin dependent diabetes mellfus (IDDM) is aiming at near-normoglycaemia. However, good glycaemic control confers at enhanced risk of hypoglycaemio. ... Frequently experienced hypoglycaemia (also nocturnal) contributes to the syndrome of hypoglycaemia unawareness and consequently to severe hypoglycaemia. In order to avoid nocturnal hypoglycaemia, we substitfuted night-time continuous subcutaneous insulin infusion (CSII) therapy in 14 patients wrth well-controlled IDDM, for the more variable bedtime NPH insulin. During a stepwise hypoglycaemic clamp (glucose nadir of 2.0 mmol/L) we studied the effect of this regimen on counterregulatory hormonal responses, warning symptoms of hypoglycaemia and cognitive dysfunction. Patients wererandomly allocated to each treatment regimen and crossed-over to the otherregimen after two months. We investigated olso the incidence of daytime hypoglycaemia and the acceptability of night-time CSII treatment. Twelve healthy volunteers, matched for age and BMI, served as controls. During the CSII treatment the awareness of hypoglycaemia improved, there was a lower frequency of hypoglycaemia with maintenance of good gycaemic control. The counterregulatory hormonal response to hypoglycaemia was improved. (Chapter 2). In the review article, the problem of nocturnal hypoglycaemia has been discussed, (Chapter 1 ). Progression of hypoglycaemia negatively alters the overal mood state. IDDM patients characterised by high scores on hostility (SCL-90 questionnaire) are more prone to experience an increase in anger (POMS questionnaire) during hypoglycaemia. (Chapter 3). Whole body 18-F fluorodeoxyglucose scintigraphy has been pertormed during hypo-and hyperglycaemia in two IDDM volunteers. (Abstracts truncated at 2000 characters)
