  • Problematika tretjih kočnikov in načrtovanje njihove pravočasne odstranitve = Problems connected with third molars and the planning of their timely extraction
Kuliš, Marko ; Kuliš, Josip ; Knez, Nino
The paper opens with an explanation of the phylogenetic theory of the reductionof facial bones, which is accompanied by a somewhat slower reduction of the number of teeth. The majority of authors ... agree that third molars are the most frequently missing teeth. Erupting third molars cause a variety of difficuties, the most serious being crowding in the dental arch. An imbalance in volume and quantitative reduction is the oldest recognized cause of malocclusion. The problem can be avoided by early diagnosis with the use of panoramic radiography and timely planned extraction of erupting third molars by the age of 18. having analysed 527 panoramic radiographs, the authors warn against premature extraction of carious first molars, which is often undertaken instead of endodontic treatment of these teeth and the extraction of erupting third molars. The late sequelae of premature extraction of first molars, illustrated in the paper with panoramic radiographs, are always a problem in prosthetic treatment. Tilted teeth in the gap preclude a stable occlusion, interfere with articulation and render prosthetic correction difficult. Problems caused by erupting third molars are not limited to pericoronitis, but may result inbruxism with consequent trauma to the temporomandibular joint.
