    Abstract - Background. With transvaginal sonographic cervical evaluation to objectify anatomic changes of the cervix in pregnant women with clinical suspicion of incompetency in second trimester of ... pregnancy. Methods. In the group of one hundred twelve singleton pregnant women with clinical suspicion of incompetent cervix or with history of premature birth in previous pregnancies ultrasound evaluation of cervical length and shape of the internalcervical os was performed. On the basis of repeated hospital gynecological examinations the group was divided into two groups: a study group with 57 women with cervical incompetence and a control group with 55 women with competent cervix. In all cases cervical evaluation was performed bytransvaginal sonographic measurement of cervical length and of eventual changes of the internal cervical os. Results. In 57 of 112 women clinical evaluation of the cervix was 1cm or less. ln this clinical suspicious group cervical length was measured in borders from 11mm to 52mm, mean cervical length was calculated 29.2mm and standard deviation 8.59. Dilatation of the internal cervical os over 5mm was found in 22 of 57 cases. In control group with 55 women cervical length was measured in borders from 21mm to 50mm, mean cervical length was calculated 36,5mm and standard deviation 6.87. Dilatation of the internal cervical os over 5mm was found in 5 cases. Conclusions. In more than half of cases in the study group cervical length values were unexpectedly high for incompetent cervix. This was also reflected by low percentage of at least minimal changes of internal cervical os shape which wasfound in 39% (22/57).
