    This article describes the follow-up of kidney transplant patients in general/family medicine. The primary care physician treating patients with end-stage renal failure has known and monitored the ... patient prior to, during and after transplantation. He is therefore involved in the psychosocial aspects of kidney transplantation and familiar with the patient's attitude towards the disease and his/her life after transplantation. Although kidney transplantation is regarded as the most natural and permanent solution, transplanted patients remain under permanent care of their nephrologists and family physicians. Changes in the patient's lifestyle and environment, alteredworking ability and permanent.follow-up require adequate adherence to therapy from the transplanted patients as well as from their families. In the team taking care of the transplanted patient (urologist and urologist's team, nephrologist and nephrologist's team, psychologist, social worker, dietitian, family physician), the primary care physician plays the role of therapy coordinator, especially between the nephrologist and the patient and his/her family. Adherence to therapy is strongly dependent upon how well the patient and his/her family have been educated. Multidisciplinary follow-up with permanent education of the patient and family, as well as continual medical education of family physicians, can improve the outcome of kidney transplantation. Better public awareness of transplantation programs and theirimportance should be our common goal as well.
