  • Kakovost zgodnje sekundarnopreventivne oskrbe bolnikov po srčnem infarktu v nekaterih slovenskih bolnišnicah = Quality of early secondary prevention care after acute myocardial infarction in some of the Slovene hospitals
Fras, Zlatko ; Jug, Borut, 1975-
Background. Slovenian guidelines for the secondary prevention and rehabilitation after a myocardial infarction propose indirect quality indicators for auditing practical applicability of individual ... reccomendations.Based on the proposed indicators, we assessed adherence to guidelines in selected Slovenian hospitals. Methods. We assessed discharge summaries of 250 consecutive patients, who had been hospitalized because of a myocardial infarction after January the 1st 2004 in five selected Slovenian hospitals (Celje, Novo mesto, Slovenj Gradec, Izola in Klinični oddelek za žilne bolezni Kliničnega centra v Ljubljani). Results. We assessed discharge summaries of 250 patients (59.9% non-STEMI; age 71.3+/-12.6 yrs, 37.9% women).Between hospitals, we observed no significant differences in drug therapy (on average, 94% patients received antiplatlet therapy, 77.2% a beta blocker and 82.2% an ACE inhibitor). However, significant inter-hospital variability (p<0.05) was observed in risk assessment (lipid measurement, echocardiographic assessment, and exercise testing), statin therapy, smoking status assessment, and followup. Conclusion. Selected hospitals differ mainly in nonpharmacological interventions (risk assessment, rehabilitation and follow-up), while no significant difference can be observed in drug treatment.
