    The mandibular molar with a C-shaped root is an anatomical variation with important implications for successful endodontic therapy. These molars have a single root with a groove on the lingual or ... buccal side, giving the pulp chamber and the root the appearance of the letter C in cross-section. The incidence in mandibular second molars is between 5 % and 32 % and depends on race. The canal system has one or more canals. If several canals are present, they are frequently connected by anastomoses. The frequency of apical deltas and lateral canals is higher than in normal mandibular second molars. Early diagnosis is essential but difficult as the radiographic appearance may mimic a regular mandibular molar. Due to the complex anatomy of the root canal system, endodontic therapy is more complicated and the success rate lower thanin normal molars. Two clinical cases of endodontic therapy involving a C-shaped mandibular molar are presented.
