    Cardiomyocyte necrosis leads to formation of akinetic fibrous scar tissue. Thus, an exogenous supply of cells with the property of forming contractile elements has been proposed as a potential ... treatment strategy to increase contractility and cardiac performance. Although several approaches to cellulartherapy for the failing heart are under investigation, the use of autologous bone-marrow stem cells has gained the most widespread clinical acceptance. In our center we developed a bone marrow stem cell transplantationprotocol that consists of three steps. First, bone marrow is stimulated with G-CSF to yield sufficient quantities of peripheral CD34 cells.Then, these cells are collected from peripheral blood via apheresis. Finally, the collected cells are administered via intracoronary route to the damaged areas of myocardium. Our first clinical experience is encouraging, with some evident stem cell therapyrelated improvements in clinical, echocardiographic and electrocardiographic parameters. By gaining additional experience in the future we hope to further improve the effectiveness of this therapeutic approach in patients with advanced heart failure.
