    There have been only few clinical studies published on the efficacy of orthoses for hallux valgus. Their results show that the permanent correction of the great toe deformation is not achievable with ... non-operative methods. The treatment is based on wide footwear with sufficient toe space and low heel in combination with insoles for foot-arch support and uniform pressure distribution. The effects of such treatment are most pronounced in patients with rheumatoid arthritis. Toe separators can temporarily reduce pain but non-weight-bearing orthoses have no proven symptomatic value and should not be prescribed routinely. In severe hallux valgus with a fixed deformation, it is not always possible to achieve the long-term pain alleviation without surgical correction. However, surgery will not have the desired effect without proper pre- and postoperative footwear adjustment. The patient should be aware that the treatment goal is to reduce foot loading, not to adjust the foot to the narrow footwear by orthotic or surgical means.
