    Clinical investigation and kidney biopsy was performed on 55 patients suffering from the endemic nephropathy the patients were grouped in three group according to their isotopic DTPA clearance rate. ... The first group of patients (12) had a normal clearance rate. The second group (16) had a clearance rate of 100-60 ml/min whereas the clearance rate of the third group (27 patients) was 60-30 ml/min. In the first group (16) had a clearance rate of hipurane was decreased as was the basic and maximum urine osmolality: urine beta 2 microglobuline was increased. These parameters were even more marked in the first and second group of patients. Pathohistological findings in the first group of patients were focal interstitial sclerosis and milt atrophy of proximal tubules epithelia focal interstitial sclerosis with glomerular and tubular changes was dominant in the second group of patients, with small arteries and arterioles showing fibroelasthosis of the blood vessels internal linings. The changes in the third group were more market. Irregular, with scarce interstitial inflammatory infiltrates. The tubular epithelium is atrophic, polymorphic with vacuole degeneration and focal necrosis. Fibroelastosis and hyalinosis of minor blood vessels was marked on certain biosysamples. The glomerular changes were not specific. The findings on the lymph vessels and veins were uncertain. Functional nephron lesions occurred earlier in relation to pathohistologic changes in endemic nephropathy.
