O Linfoma de Hodgkin é uma neoplasia linfoide monoclonal derivada na maioria dos casos das células B, composta por células de Hodgkin mononucleares e Hodgkin Reed-Sternberg (HRS) multinucleadas ...presentes em infiltrados contendo uma mistura variável de pequenos linfócitos não neoplásicos, eosinófilos, neutrófilos, histiócitos, células plasmáticas, fibroblastos e fibras de colágeno. Pode ser dividida em quatro subtipos histológicos: esclerose nodular, celularidade mista, rico em linfócitos e depleção linfocitária, sendo a esclerose nodular o subtipo mais frequente.Ocorre principalmente em adultos jovens. O padrão ouro para o diagnóstico é anatomia patológica e imunohistoquímica da biópsia do linfonodo. No entanto, a citometria de fluxo multiparamétrica de alta sensibilidade é uma ferramenta diagnóstica importante que permite uma avaliação rápida da expressão do antígeno celular para fornecer rapidamente informações imunofenotípicas adequadas para detecção da neoplasia.
Apresentar um relato de caso de Linfoma de Hodgkin subtipo esclerose nodular diagnosticado a partir de biópsia de linfonodo. Relato de caso: Paciente masculino, 30 anos, procurou serviço de oncologia referindo linfonodomegalia cervical há aproximadamente 1 ano, com acentuada progressão nos últimos 2 meses e dor em região de ouvido esquerdo. Queixava-se de prurido difuso, perda ponderal de 20 kg, sudorese noturna e febre eventual. O exame físico mostrou conglomerado linfonodal bilateral medindo aproximadamente 10 cm, além de múltiplas pápulas e lesões de escoriação decorrente do prurido. Dois dias após a consulta, internou para realizar exames de imagem, laboratoriais e biópsia de linfonodo. Apresentava discreta anemia, leucócitos e plaquetas normais, LDH aumentada, função hepática e renal normais. A tomografia evidenciou linfonodomegalias mediastinais e cervicais bilaterais. A imunofenotipagem da biópsia de linfonodo mostrou 0,33% de células anormais de tamanho e complexidade muito altos, com forte expressão para os antígenos de membrana CD3, CD15, CD30, CD40, CD45, CD95 e fraca expressão de CD20. Relação de linfócitos CD4/CD8=6,6. A imunohistoquímica mostrou células atípicas com positividade para CD15, CD30, PAX5 (fraco), Ki67 (40%) e negatividade para EBV. Os resultados apresentaram quadro histológico e perfil imunofenotípico compatíveis com Linfoma de Hodgkin, subtipo esclerose nodular. Após a realização dos exames o paciente foi encaminhado para o serviço de hematologia, onde recebeu o diagnóstico e foi orientado sobre o prognóstico e importância do tratamento. Realizou exames complementares e de estadiamento; sorologias negativas (HIV, hepatite B e C, dentre outras); PET evidenciou linfonodos supra e infradiafragmáticos compatível com neoplasia linfoproliferativa. Na sequência o paciente iniciou o primeiro ciclo de quimioterapia (protocolo ABVD). Retornou para consulta após 25 dias referindo melhora significativa e boa tolerância ao tratamento, sem sintomas B e demais queixas. Recebeu prescrição de dois novos ciclos de quimioterapia e segue em acompanhamento.
A imunofenotipagem por citometria de fluxo é uma ferramenta importante que permite diagnóstico rápido e consistente no diagnóstico do Linfoma de Hodgkin. A associação da citometria de fluxo com a análise morfológica e a imunohistoquímica permitiu a conclusão do relato de caso em amostra de biópsia de linfonodo.
The design and construction of a PET camera module with high sensitivity, full 3-D spatial reconstruction and very good energy resolution is presented. The basic principle consists of an axial ...arrangement of long scintillation crystals around the Field Of View (FOV), providing a measurement of the transverse coordinates of the interacting 511
keV gamma ray. On top of each layer of crystals, an array of Wave-Length Shifter (WLS) strips, which collect the light leaving the crystals sideways, is positioned orthogonal to the crystal direction. The signals in the WLS strips allow a precise measurement of the
z (axial) co-ordinate of the 511
keV γ-ray gamma impact. The construction of two modules used for demonstration of the concept is described. First preliminary results on spatial and energy resolution from one full module will be shown.
In the ideal crack model in eddy-current testing frame, the field-flaw is equivalent to a current dipole layer on its surface. This model has shown its efficiency, as well for the computing accuracy, ...as for the CPU time. The goal of this paper is to improve this model taking into account the inclination, the conductance, and the low thickness of the crack.
Standard Positron Emission Tomography (PET) cameras need to reach a compromise between spatial resolution and sensitivity. To overcome this limitation we developed a novel concept of PET. Our AX-PET ...demonstrator is made of LYSO crystals aligned along the
z coordinate (patient's axis) and WLS strips orthogonally placed with respect to the crystals. This concept offers full 3D localization of the photon interaction inside the camera. Thus the spatial resolution and the sensitivity can be simultaneously improved and the reconstruction of Compton interactions inside the detector is also possible. Moreover, by means of G-APDs for reading out the photons, both from LYSO and WLS, the detector is insensitive to magnetic fields and it is then suitable to be used in a combined PET/MRI apparatus. A complete Monte Carlo simulation and dedicated reconstruction software have been developed. The two final modules, each composed of 48 crystals and 156 WLS strips, have been built and fully characterized in a dedicated test set-up. The results, obtained with a
22Na point source (0.25
mm diameter), of the single module performances and a first estimation of the performances with the two module system are reported.
