The SAGES Manual on the Fundamental Use of Surgical Energy (FUSE) emphasizes good communication and promotes best practice for the use of electrosurgical, ultrasonic, and microwave energy sources in ...the operating theatre. This manual describes the basic technology of energy sources in the operating room and demonstrates the correct use and indications of energy sources in clinical practice. It also addresses the potential complications, hazards, and errors in the use of surgical energy sources and evaluates the potential interactions of energy sources with other medical devices. Any healthcare professional who has ever picked up an energy device in the OR such as a 'Bovie' , Ultrasonic or bipolar instrument will better understand how it works, when to apply it, and what are the possible hazards and errors in its use. The SAGES Manual on the Fundamental Use of Surgical Energy (FUSE) is the first volume of its kind to provide such guidance and will be of great value to surgeons, anesthesiologists, nurses, endoscopists, and allied health care professionals who use these devices.
Abstract We describe a case of a 15-year-old girl who presented with 2 painful masses in her right breast. Ultrasound confirmed the presence of 2 lesions, both of which appeared noncharacteristic for ...fibroadenomas. Both lesions were surgically resected. One was found to be a fibroadenoma and the other a granular cell tumor, both benign upon further histologic evaluation. Breast masses are rare in the pediatric population. The finding of a concurrent fibroadenoma and granular cell tumor is unique and has not been previously reported. Granular cell tumors of the breast are relatively uncommon. Often, they are mistaken for a breast malignancy. The concerning clinical and radiographic findings in this patient warranted operative excision.
Despite increases in the scientific evidence for a variety of medical treatments, a gap remains in the adoption of best medical practices. This manuscript describes a process for adapting published ...summary guides from comparative effectiveness research to render them concise, targeted to audience, and easily actionable; and a strategy for disseminating such evidence to patients and their physicians through a web-based portal and linked electronic health record. This project adapted summary guides about oral medications for adults with type 2 diabetes to a fifth-grade literacy level and modified the resulting materials based on evaluations with the Suitability Assessment of Materials instrument. Focus groups and individual interviews with patients, diabetes providers, and health literacy experts were employed to evaluate and enhance the adapted summary guide. We present the lessons learned as general guidelines for the creation of concise, targeted, and actionable evidence and its delivery to both patients and providers through increasingly prevalent health information technologies.

Technological advancements, along with economic and political issues, have resulted in major changes in surgical education. The development of high fidelity simulators and the widespread availability ...of the Internet have allowed learning to be shifted away from the operating room. Furthermore, the Internet provides an opportunity for surgical educators to standardize general surgery training and assessment and to develop collaborations nationally and globally. This paper highlights presentations about the challenges as well as the rewards of surgical education in the age of the Internet from the 2009 Academic Surgical Congress.
Most energy devices can be used safely in pediatric patients, but surgeons must be aware of the unique anatomic and physiologic considerations that affect their use. Smaller anatomic structures and ...pediatric instruments can concentrate current, and the proximity of low- and high-resistance tissues, monitoring devices, and surgical instruments can lead to unpredictable electrical pathways. The variations seen in patients provide an excellent model for thinking about energy-device safety in any patient with altered body composition.
