The Southern Caucasus lies at the intersection of Africa, the Levant and Eurasia, and is thus a region of considerable interest in the study of Pleistocene hominin population dynamics and behaviour. ...While Palaeolithic archaeological sites in the region such as Dmanisi and Nor Geghi 1 attest to such palaeogeographic significance, a greater understanding of the chronology and nature of climatic and geomorphic changes in the region is needed to fully understand hominin settlement dynamics. The Hrazdan river valley, central Armenia, has the potential to offer such insights given its rich Palaeolithic record and complex history of Pleistocene infill as a result of alluvial, lacustrine, aeolian, and volcanic processes. We therefore present a stratigraphic framework for basin infill and hominin activity during the Pleistocene, based on extensive geomorphological and geological mapping, published chronometric results (40Ar/39Ar and K-Ar), and archaeological survey. We demonstrate that the onset of Pleistocene volcanism in the Gegham Range to the immediate east of the Hrazdan valley occurred around 700 ka BP, after which there were several phases of effusive eruption lasting until 200 ka. Interbedded with lava emplaced by these eruptions are alluvial and lacustrine sequences, some with evidence of pedogenesis and several of which have yielded Palaeolithic artefacts. Taken together these sequences suggest a cyclical model of infill whereby lava flow along the valley resulted in the blockage of the palaeo-Hrazdan river and lake formation in the lea of the lava dams. Breaching of these dams resulted in a shift to predominately fluvial deposition, and the consequent development of floodplain soils. Hominin populations exploited the floodplains at times when the last of these phases coincided with interglacial and interstadial climates, but they also occupied the surrounding valley sides during the same warm, humid phases.
•Development of a stratigraphic framework for volcanism, landscape development and Palaeolithic occupation in the Central Armenian Highlands.•Demonstrates several phases of effusive and explosive volcanic activity during the Middle and Late Pleistocene.•Landscape change was caused by a combination of volcanic activity, hydrological development and climate change.•Preservation of stratified Palaeolithic sequences indicates that hominins were exploiting floodplain and lake environments during warm episodes.•Close association of sedimentarysequences and volcanic products provides a rare opportunity for precisely dating Palaeolithic occupations.
L’hyperglycémie sans cétone peut être responsable de crises partielles à point de départ occipital avec des anomalies radiologiques à l’IRM cérébrale à type d’hyposignal T2 au niveau de la substance ...blanche.
Nous rapportons le cas d’un patient de 47 ans aux antécédents d’hépatite C traitée qui a commencé à présenter 2 mois avant son hospitalisation une amputation du champ visuel à droite avec des troubles visuels à type de flashs lumineux paroxystiques et des épisodes d’hémiparésie droite fluctuante avec à l’examen clinique une hémianopsie latérale homonyme droite isolée. La glycémie à jeun est à 4g/L sans cétonurie. L’ HbA1c est à 16.2 %. L’EEG est dépourvu d’anomalie épileptique paroxystique ou de foyer. L’IRM cérébrale montre un hypersignal en diffusion au niveau occipital et pariétal gauche avec baisse de l’ADC, un hyposignal FLAIR prédominant au niveau de la substance blanche avec épaississement du ruban cortical, sans prise de contraste pathologique. Un traitement antiépileptique par lévétiracétam associé à une insulinothérapie et un antidiabétique oral est introduit, permettant une amélioration des glycémies et une disparition des anomalies visuelles paroxystiques mais la persistance d’une hémianopsie latérale homonyme droite. L’IRM cérébrale de contrôle à 2 mois retrouve la persistance d’anomalies en pariéto-occipital gauche au niveau de la substance blanche en hypersignal FLAIR, diffusion, et hyposignal T1 sans atteinte corticale.
Quelques cas de crises partielles occipitales dans un contexte d’hyperglycémie sans cétone sont décrits dans la littérature. Les patients présentaient des troubles visuels, des anomalies radiologiques régressives après l’instauration du traitement avec un EEG pathologique. Il s’agit du premier cas décrit avec un EEG sans anomalie, des séquelles radiologiques et des séquelles visuelles favorisées par le retard diagnostique et thérapeutique.
L’épilepsie partielle occipitale dans l’hyperglycémie sans cétone est une urgence diagnostique et thérapeutique permettant d’éviter des séquelles neurologiques visuelles définitives.
Мета роботи – продемонструвати потреби студентів щодо оновлення формату проведення лекційних занять і можливі шляхи її реалізації.
Основна частина. Вимогою сьогодення є підготовка нових лікарів як ...компетентних, мобільних, здатних вдосконалюватися та швидко орієнтуватися у сучасному світі науки і медицини. Зміна поколінь привела до зниження ефективності застарілих навчальних підходів. Це диктує потребу в оновленні навчальних програм і впровадженні інтерактивних, більш гнучких підходів до навчання. Нами було проведено анонімне анкетування 2139 студентів 3–6 курсів медичних факультетів Національного медичного університету імені О. О. Богомольця, яке дало можливість оцінити ставлення сучасних студентів до лекцій.
Загалом студенти досить високо оцінюють користь від лекційних занять, більшість опитаних вважає, що отримує з лекцій близько 60 % навчального матеріалу. Більшість студентів, які взяли участь в анкетуванні, висловила позицію, що усні лекції з клінічних дисциплін потрібні, але потребують істотної зміни формату. Існуючий формат лекцій задовольняє лише 14 % опитаних студентів. Основними недоліками існуючого формату лекцій студенти вважають повторення інформації з підручників/посібників і брак конкретних клінічних прикладів.
Висновки. Оптимальним форматом лекцій для клінічних кафедр студенти вважають такий: загальна тривалість лекції має бути 45 хв і включати 2 блоки по 20 хв із 5-хвилинною перервою між ними. В одному блоці повинна міститися сучасна інформація щодо клінічної симптоматики, діагностики і лікування захворювань, в другому – розбір клінічних випадків з прикладами формулювання діагнозів, планом обстеження і лікування.
This study is limited by the fact that each hospital has different costs for staffing as well as ambiguous billing patterns that make it difficult to correctly appreciate the value of perioperative ...staffing and costs for the hospital, insurance company, and the patient. Additional costs, as noted above, that were not included in this study would be also added to the potential cost savings. Therefore, our results may in fact appreciate the true difference in costs between the operating room and treatment room anesthesia. As anesthesia providers, we are involved in the perioperative care of surgical patients. We must continuously improve our perioperative care to enhance patient safety while increasing efficiency and decreasing costs. Using regional anesthesia to minimize main operating room times may be an acceptable approach to achieving cost saving measures, as well as reducing unnecessary staffing and main operating resources.
