The most severe effects of rheumatoid arthritis (RA) are loss of physical function and chronic pain, which may have a major impact on different areas of the person's existence. The aim of this study ...was to get an insight into the quality of life (QOL) in subjects with RA in connection with pain perception and functional ability. The following instruments were used: the World Health Organization Quality of Life Questionnaire (WHOQOL-BREF), Short Form Health Survey (SF-36), Health Assessment Questionnaire Disability Index (HAQ-DI) and Visual Analog Scale for Pain (VAS Pain). The results indicated that there was no statistical difference in the QOL between subjects with RA and healthy population according to SF-36 Croatian norms. Also, the results showed that stronger pain experience was significantly associated with poorer social functioning assessment (SF36SF, Spearman's rho=-0.463, p<0.05), poorer general health perception (SF36GH, Spearman's rho=-0.432, p<0.05) and poorer physical functioning (WHOPH, Spearman's rho=-0.688, p<0.01). Furthermore, the subjects evaluating their general functional state worse were found to have worse physical functioning (SF36PF, Spearman's rho=-0.699 and WHOPH, Spearman's rho=-0.769), poorer social functioning (SF36SF, Spearman's rho=-0.580) and experienced greater pain intensity (SF36BP, Spearman's rho=-0.652). Therefore, additional efforts should be invested to define a holistic and integrative model of treatment and rehabilitation of people with RA, focused on pain relief, improvement of functional ability, encouraging social interaction and supporting positive emotional responses.
Žarište istraživanja bilo je na proučavanju povezanosti vizualno-motoričke integracije i koordinacije s razvojem vještine crtanja čovjeka u djece predškolske dobi prosječnog razvoja. Uzorak od 28 ...ispitanika podijeljen je na eksperimentalnu i kontrolnu skupinu. Eksperimentalna je skupina sudjelovala u programu poticanja psihomotoričkog razvoja, a kontrolna je skupina bila u redovitome predškolskom programu. Rezultati su pokazali kako je program poticanja psihomotoričkog razvoja utjecao na bolje rezultate u vizualno-motoričkoj integraciji i koordinaciji. Istaknula se i značajna povezanost vizualno-motoričke integracije i bilateralne koordinacije sa crtežom čovjeka u eksperimentalnoj skupini. Navedeni rezultati upućuju na važnost daljnjega proučavanja programa poticanja i njihove uloge u pedagoškoj praksi sa svrhom pozitivnog utjecaja na razvojni profil djeteta predškolske dobi.
Assistive technology is a collective term that refers to assistive devices and services for people withdisabilities. The use of assistive technology in working with people with disabilities ...facilitates their learning and performance of activities of daily living. The knowledge and experience of experts is necessary for the selection, purchase, and use of assistive technology. For the purpose of this work, an online survey was conducted among experts in Bosnia and Herzegovina who had the opportunity to use and apply assistive technology through their work. The research participants (N=23) were educational rehabilitators, speech therapists, teachers, pedagogues, physiotherapists, and nurses. The online questionnaire consisted of 33 open and closed-ended questions. The data obtained were processed using quantitative and qualitative data processing methods. The aim of the research was to ask experts in Bosnia and Herzegovina about their experience with the use of assistive technology, their knowledge and attitudes, and the challenges they face in their work. The results show that assistive technology has not beenused for long in Bosnia and Herzegovina and that experts encounter many challenges in their work, such as financial problems, lack of technical support, and difficulties in working with parents. They also emphasize the need for more training in assistive technology.Keywords: assistive technology, experts, experience, knowledge, education.
There are many risk factors from the biological or psychosocial context that may adversely affect developmental outcomes. 'Numerous studies have shown that good interaction between parent and child ...can reduce the impact of biological and psychosocial factors on child development. Through interaction with parents, the child is getting acquainted with the world around you, and if the connection between child and parent is not set on solid foundations, child's development is threatened, regardless what the reasons for the poor interaction are (child's health, parental psychopathology, parental stress, environmental factors, etc.). Optimal interaction between parent and child can be particularly challenged in families where there are multiple riskfactors. The aim of this study was to identify biopsychosocial factors of the mother and child that have an influence on parental stress, marriage satisfaction, the level of psychopathological symptoms, mothers, levels of perceived stress and maternal assessment of child's self-regulation. Factors that were studied were maternal age at birth, problems during pregnancy or childbirth, maternal employment, years of education of mother, child nutrition, mode of delivery, the presence of neurodevelopmental risks in children, diagnosis of the existence of difficulties in the development of the child, child's birth weight below 2500 grams, gestational age below 37 weeks, gender of the child and the child's age. The results show that the existence of different risk factors has a different impact on the area. The largest impact on the area had: the existence of neurodevelopmental risk factors in a child, the existence diagnosis of the child and low birth weight. High parental stress and poor self-regulation assessment of the child appeared to be possible predictors of poor interaction beween mother and child, especially recognised at children with neurodevelopmental risks and / or developmental disabilities. This survey yielded insights about the impact of biopsychosocial factors on the observed interaction of mother and child, as well as on the mother's perception of themselves, their children andfamilies. Some of the factors that may contribute to the quality of early intervention and quality of life for children and families where risk factors are present were identified. Application of the proposed instruments for assessment, evaluation method and analysis of contemporary approaches to research in the world will enable the development of new methods for multidimensional analysis of biological, psychological and social impact on the early interaction between mother and child and its application in early developmental intervention programs in educational rehabilitation. Key words: early mother-child interaction, biopsychosocialfactors, quality of life Postoji mnogo rizicnih cimbenika iz bioloskog ili psihosocijalnog konteksta koji mogu nepovoljno utjecati na razvojni ishod. Brojna istrazivanja su pokazala da dobra interakcija roditelja i djeteta moze smanjiti utjecaj bioloskih i psihosocijalnih cimbenika na razvoj djeteta. Kroz interakciju s roditeljima dijete upoznaje svijet oko sebe i ako veza izmedu djeteta i roditelja nije postavljena na zdravim temeljima, razvoj djetetaje ugrozen, bez obzira koji su razlozi za losu interakciju (djetetovo zdravstveno stanje, psihopatologija roditelja, roditeljski stres, okolinski cimbenici i sl.). Optimalna interakcija izmedu roditelja i djeteta moze biti osobiti izazov u obiteljima kod kojihpostoje visestruki cimbenici rizika. Cilj ovoga istrazivanja bio je utvrditi biopsihosocijalne cimbenike kod majke i djeteta koji imaju utjecaj na roditeljski stres, zadovoljstvo brakom, razinu psihopatoloskih simptoma majke, razinu percipiranog stresa te majcinu procjenu djetetove samoregulacije. Cimbenici koji su proucavani su majcina dob pri porodu, teskoce tijekom trudnoce ili poroda, zaposlenje majke, godine obrazovanja majke, nacin prehrane djeteta, nacin poroda, postojanje neurorazvojnih rizika kod djeteta, postojanje dijagnoze poteskoca u razvoju kod djeteta, porodajna masa djeteta ispod 2500 grama, gestacijska dob djeteta ispod 37 tjedana, spol djeteta i dob djeteta. Dobiveni rezultati pokazuju da postojanje razlicitih cimbenika rizika ima razliciti utjecaj na promatrana podrucja. Najveci utjecaj na promatrana podrucja imali su: postojanje neurorazvojnih cimbenika rizika kod djeteta, postojanje dijagnoze djeteta i niska porodajna masa djeteta. Visoki roditeljski stres i losa procjena samoregulacije djeteta pokazali su se kao moguci prediktori lose interakcije majke i djeteta, osobito djetetas neurorazvojnim rizicima i/ilipoteskocama u razvoju. Ovim istrazivanjem dobivene su spoznaje o utjecaju promatranih biopsihosocijalnih cimbenika na interakciju majke i djeteta, kao i na majcinu percepciju sebe, svoga djeteta i obitelji. Utvrdeni su neki od cimbenika koji mogu pridonijeti kvaliteti rane intervencije i kvalitete zivota djece i obitelji kod kojih suprisutni cimbenici rizika. Primjena predlozenih instrumenata procjene, nacin evaluacije i analiza suvremenih pristupa u istrazivanjima u svijetu omoguciti ce razvijanje novih multidimenzionalnih metoda u analizi bioloskih, psiholoskih i socijalnih utjecaja na ranu interakciju majke i djeteta iprimjenu uprogramima rane razvojne intervencije u edukacijskoj rehabilitaciji. Cilj: Ispitivanje povezanosti kvalitete spavanje i kvalitete života u žena oboljelih od raka dojke, te razlika u doživljaju kvalitete spavanja i kvalitete života u odnosu na dob, vrijeme proteklo od ...postavljanja dijagnoze i vrstu operativnog zahvata. Ispitanici i metode: Istraživanjem je obuhvaćen uzorak od 96 ispitanica oboljelih od raka dojke. Jednokratno je primijenjen Pittsburgh Sleep Quality Index (PSQI) konstruiran za procjenu kvalitete spavanja u onkoloških bolesnika i FACIT Measurement System - verzija FACT-B konstruirana za populaciju osoba s rakom dojke u svrhu procjene fizičkih, socijalnih, emocionalnih, funkcionalnih i dodatnih determinanti kvalitete života. Rezultati: U uzorku žena obuhvaćanih ovim istraživanjem prisutna je tendencija prema lošijoj kvaliteti spavanja, te nije utvrđena razlika u kvaliteti spavanja u odnosu na kronološku dob, vrijeme proteklo od postavljanja dijagnoze i vrstu oparativnog zahvata. Isto tako, nije dobivena statistički značajna razlika u procjeni kvalitete života u odnosu na vrijeme proteklo od dijagnoze i vrstu operativnog zahvata. U odnosu na kronološku dob utvrđena je statistički značajna razlika između ispitanica u dobi od 31-40 godina i ispitanica u dobi 51-60 godina (p=0,038) pri čemu su ispitanice mlađe dobne skupine imale lošiju kvalitetu života. Također je dobivena statistički značajna povezanost između kvalitete spavanja i kvalitete života (p=0,008), odnosno žene koje su imale lošiju kvalitetu spavanja imale su statistički značajno nižu kvalitetu života. Zaključak: Dobiveni rezultati ukazuju na potrebu provođenja daljnjih istraživanja s ciljem ispitivanja kauzalnih odnosa između kvalitete spavanja i kvalitete života te koncipiranja adekvatnih medicinskih, psihoterapijskih i komplementarno-suportivnih intervencija usmjerenih na ublažavanje psihofizičkog distresa u oboljelih od raka dojke.
Quality of life has become an important research subject in different disciplines. The objective of this research is to examine the metric characteristics of the questionnaire EORTC QLQ-C30 (The ...European Organization for Research and Treatment of Cancer’s Quality of Life Questionnaire) and to determine differences in the quality of life of people with and without malignant diseases. The assessment of the quality of life included two groups of interviewers: in the first group were adults with breast cancer, colon cancer and head/neck cancer (n=54), and in the second group there were adults without malignant diseases (n=266). Metric characteristics of the questionnaire were checked with the Rtt7 Programme, and for determination of differences between groups, robust discrimination analysis was used.
Compared with the people without malignant diseases, patients have assessed their condition as better in terms of physical, business and emotional functioning, with some symptoms of fatigue and pain. Furthermore, patients have assesses their social functioning as worse, and stressed more symptoms of nausea and more financial difficulties as a result of disease. Patients have assessed overall quality of life and health condition as worse than the interviewers without diseases. Women with malignant diseases and women without diseases have assessed their business, physical and emotional functioning as worse, more symptoms in all life quality variables and more financial difficulties compared with men in the same group. However, women without diseases have assessed their overall life quality as better than the men in the same group. Women with breast cancer have assessed their quality of life in almost all variables as worse compared with the interviewers with other types of malignant diseases, while men with head/neck cancer have assessed their overall quality of life and health condition significantly better than women with breast cancer and patients with colon cancer.
Gathered result show that patients assess their life quality as satisfying, however, in some areas, together with medical assistance, they need continuous psychosocial support, particularly women with breast cancer.
Early intervention is the process of informing, consulting, training and support to children at early age at which the state of the possible variations in the development of a high risk for further ...development, as well as their families, is determined. This process includes children at risk factor for developmental deviations that would later have an impact on their future development. Early intervention program can be provided in a variety of environments, but the focus is on the natural environment of the child. It turned out that the program is the most effective if the early intervention began immediately after determining deviations or disclosure of a disability. The main objective of this paper is to present educational and rehabilitation programs, which objective is to foster cognitive, emotional, social, motor, and communication skills of children with multiple disabilities in the early-integration program. The program was conducted in the family -with twins with West syndrome chronological, age 2.5 years, under the supervision of the Cabinet in early psychomotoric stimulation of the Rehabilitation Centre for Education and Rehabilitation Sciences, during the academic year 2010-2011. Early developmental integration program, that was implemented, included the basic elements of neuro-developmental treatment (hereinafter: NDT), sensory integration programs, cognitive elements of development (Piaget's theory of cognitive development and promotion of functional learning), elements of Dennison educational kinesiology, the components of speech and language, orofacial stimulation and support to the family. Procedures for the assessment and evaluation used Munich Developmental Diagnostic and Gunzburg test whose components are used as variables of the study. As a data processing method was used INDIF component analysis through a series of equally distant time points. Taking into account the statistical data, it can be concluded that the early developmental integration program had a positive impact on the development of girls. In order fully to be able to understand their development needs to take into account the genetic predisposition and the environment in which they grow up. Keywords: early intervention, West syndrome, Early developmental integration program, INDIF component analysis Rana intervencija je proces informiranja, savjetovanja, edukacije i podrske djeci u ranoj dobi kod koje je utvrceno stanje moguceg odstupanja u razvoju s visokim rizikom za daljnji razvoj, kao i njihovim obiteljima. Ovaj proces obuhvaca i djecu kod koje postoji cimbenik rizika za razvojno odstupanje koje bi kasnije moglo utjecati na njihov daljnji razvoj. Program rane intervencije moze se pruzati u razlicitim okruzenjima, no naglasak se stavlja na prirodno okruzenje djeteta. Pokazalo se da je program rane intervencije najefikasniji ako se s njim pocne odmahpo utvrcivanju odstupanja ili otkrivanju teskoce u razvoju. Osnovni cilj ovog rada je prikaz edukacijsko-rehabilitacijskog programa usmjerenog poticanju kognitivnih, emocionalnih, socijalnih, motorickih, komunikacijskih i govornih vjestina djece s visestrukim teskocama u okviru Ranog razvojnog integracijskog programa za dijete s visestrukim teskocama koji se provodi u Kabinetu za ranu psihomotoricku stimulaciju Centra za rehabilitaciju Edukacijsko-rehabilitacijskogfakulteta Sveucilista ua Zagrebu. Program se provodio u obitelji blizanki s West sindromom kronoloske dobi 2,5 godine uz superviziju Kabineta za ranu psihomotoricku stimulaciju Centra za rehabilitaciju Edukacijsko- rehabilitacijskog fakulteta, kroz akademsku godinu 2010./2011. Rani razvojni integracijski program koji se provodio obuhvacao je osnovne elemente neurorazvojnog tretmana (NDT), programa senzorne integracije, kognitivne elemente razvoja (Piagetova teorija poticanja kognitivnog razvoja i funkcionalno ucenje), Dennisonovu edukinesteticku metodu, komponente govora i jezika, orofacijalnu stimulaciju te podrsku obitelji. Za postupke procjene i evaluacije koristena je Miinchenska funkcionalna razvojna dijagnostika i Gunzburg test cije komponente se koriste kao varijable ovog istrazivanja. Kao metoda obrade podataka koristena je INDIF komponentna analiza stanja ispitanika kroz niz jednako udaljenih vremenskih tocaka. Uzimajuci u obzir statisticke pokazatelje, moze se zakljuciti da je Rani razvojni integracijski program imao pozitivan utjecaj na razvoj djevojcica. Kako bi u potpunosti mogli razumjeti njihov razvoj treba uzeti u obzir genetsku predispoziciju te okruzenje u kojem odrastaju. The article presents the overview of few theoretical approaches and early intervention models which are implemented in Croatia and in the world with the emphasis on the early intervention programmes ...effects on the high-risk children for developmental motor disorders. The overview of some early intervention programmes with regard to beginning of the intervention (during and after the exit from the department for neonatal care) is presented. Researches have shown that there is distinction between intervention models for the children horn at term and early-born children. Results of the intervention programmes implementation on the motoric development have shown that the impact have only those programmes into which were included also the families of the children and which were conducted in the child's home. There in no sufficient information on the effects of the early intervention impact on the day-to-day activities and socialisation in the school age, particularly of the positive effects of early intervention on motoric development. During the conduction and evaluation of the early intervention programme, the need for identification of all child's specifities, but, at the same time, utilisation of all available potentials of the child and other family members and local community, is determined. The article gives overview of the work of the Cabinet for early psychomotoric stimulation which is established at the Centre for rehabilitation of the Faculty for education and rehabilitation, University of Zagreb. The Cabinet was among first nonmedical (paramedical) departments in Croatia which promoted the need for early intervention implementation in children with neurodevelopment risks, support to the whole family and necessity of early intervention implementation in the local community. Key words: early intervention, neurodevelopmental risks, motor development Sazetak: U clanku se daje prikaz nekoliko teorijskih pristupa i modela rane intervencije koji se provode u Hrvatskoj i svijetu s naglaskom na prikaz ucinaka ranih interventnih programa kod djece visokorizicne za odstupanja u motorickom razvoju. Dat je pregled nekih ranih interventnih programa s obzirom na vrijeme zapocinjanja intervencije (tijekom ili nakon izlaska s odjela neonatalne intenzivne skrbi). Istrazivanja su pokazala da se razlikuju modeli intervencije za djecu rodenu u terminu i prijevremeno rodenu djecu. Rezultati primjene interventnih programa na motoricki razvoj pokazali su da dugotrajni ucinak imaju samo oni programi u koji su, osim djece, ukljucene i njihove obitelji i koji se provode u domu djeteta. Postoji nedovoljno informacija o ucincima rane intervencije na aktivnosti svakodnevnog zivota i socijalizaciju u skolskoj dobi, a kao eventualni rezultat pozitivnog ucinka rane intervencije na motoricki razvoj. Prilikom provodenja i evaluacije programa rane intrevencije, utvrdena je potreba identificiranja svih posebnosti djeteta, ali istovremeno i koristenja svih raspolozivih potencijala, kako samoga djeteta, tako i ostalih clanova obitelji i lokalne zajednice. U clanku je dat i prikaz rada Kabineta za ranu psihomotoricku stimulacija 1997. godine pri Centru za rehabilitaciju Edukacijsko-rehabilitacijskog fakulteta Sveucilista u Zagrebu. Kabinet je bio medu prvim nemedicinskim (paramedicinskim) odjelima u Hrvatskoj u okviru koji je promovirao potrebu provodenja rane intervencije kod djece s neurorazvojnim rizicima, podrske cijeloj obitelji i nuznost utemeljenja rane intervencije u lokalnoj zajednici.
