Resumo: O objetivo do trabalho foi estimar fatores associados ao bem-estar psicológico de travestis e mulheres transexuais. Estudo transversal com 602 travestis e pessoas transexuais em sete ...municípios do Estado de São Paulo, Brasil entre 2014 e 2015. Foi realizada seleção amostral com abordagem consecutiva e técnica bola de neve. A variável dependente foi bem-estar psicológico (WHOQOL-BREF) e as independentes foram: características sociodemográficas, modificações corporais, condições de saúde, violência e encarceramento. A análise de variância múltipla foi usada para identificar os fatores associados. A maioria tinha cor da pele preta ou parda e entre 25 e 39 anos de idade, até o Ensino Médio completo, renda individual de até dois salários mínimos e trabalhava, sendo 42,3% profissionais do sexo. Cerca de um quarto já foi presa. Em torno de um quarto fazia tratamento para HIV. O escore médio observado foi de 63,2 (IC95%: 61,8-64,6). Na análise múltipla, estiveram associados ao menor bem-estar psicológico: não ter endereço fixo, ter menor escolaridade, estar insatisfeita com as relações pessoais, suporte de amigos ou procedimentos transexualizadores realizados e ter sofrido violência verbal ou sexual. Enquanto piores condições de vida e de exposição à violência prejudicam o bem-estar psicológico de travestis e mulheres transexuais, a possibilidade de realizar transformações corporais desejadas e o respeito ao nome social interferem positivamente na avaliação que fazem de suas vidas.
Abstract: This article sought to estimate factors associated with the psychological well-being of transvestites and trans women. The dependent variable was psychological well-being (WHOQOL-BREF) and the independent variables were: sociodemographic characteristics, body modifications, health conditions, violence and incarceration. We used a multiple variance analysis to identify associated factors. Most were black or brown and were aged between 25 and 39 years, had up to complete secondary education, individual income of up to two times the minimum wage and worked, and 42.3% were sex workers. Around one-quarter had been incarcerated. Around one-quarter were in treatment for HIV. Mean psychological well-being score was 63.2 (95%CI: 61.8-64.6). In the multiple analysis, the factors associated with lower psychological well-being were: not having a fixed address, having lower educational levels, being dissatisfied with personal relationships, friend support or the gender-affirming procedures they had undergone and having suffered verbal or sexual violence. While worse living conditions and exposure to violence harm the psychological well-being of transvestites and trans women, the possibility of undergoing desired body transformations and having their social name respected interfere positively in their evaluations of their lives.
Resumen: El objetivo fue estimar factores asociados al bienestar psicológico de travestis y mujeres transexuales. Se trata de un estudio transversal con 602 travestis y personas transexuales en siete municipios del estado de São Paulo, Brasil, entre 2014 y 2015. Se realizó una selección de muestras con un enfoque consecutivo y técnica bola de nieve. La variable dependiente fue bienestar sicológico (WHOQOL-BREF) y las independientes fueron: características sociodemográficas, modificaciones corporales, condiciones de salud, violencia y encarcelamiento. Se usó un análisis multivariante de variancia para identificar los factores asociados. La mayoría tenía el color de piel negro o eran mulatos, y entre 25 y 39 años de edad, con hasta la enseñanza media completa, renta individual de hasta dos salarios mínimos y trabajaba, siendo un 42,3% profesionales del sexo. Cerca de un cuarto ya estuvo presa. También cerca de un cuarto estaba con tratamiento para el VIH. Las puntuaciones medias de bienestar sicológico fue 63,2 (IC95%: 61,8-64,6). En el análisis múltiple, estuvieron asociados a un menor bienestar sicológico: no tener dirección fija, tener menor escolaridad, estar insatisfecha con las relaciones personales, apoyo de amigos o procedimientos transexualizadores realizados, y haber sufrido violencia verbal o sexual. Mientras que unas peores condiciones de vida y de exposición a la violencia perjudican el bienestar sicológico de travestis y mujeres transexuales, la posibilidad de realizar las transformaciones corporales deseadas y el respeto al nombre social interfieren positivamente en la evaluación que hacen de sus vidas.

We performed a narrative review of the literature in which we analyzed the various ...conceptions on frequent testing over time, the implications for health programs, and the main social markers that influence the incorporation of HIV testing as routine care. Although it has existed since the 1990s, recurrent testing among MSM was frequently interpreted as increased exposure to HIV due to lack of condom use, and therefore as "unnecessary" testing. Beginning in the 2000s, periodic testing has become a programmatic recommendation and has been interpreted as a goal. Individuals' perception of their use of the test has rarely been considered in order to characterize such use as routine care. On the social and cultural level, individual aspects associated with recent or routine testing were included in contexts of favorable norms for testing and less AIDS stigma. Differences in generation, schooling, and types of affective-sexual partnerships play an important part in testing. Such differences highlight that the epidemiological category "men who have sex with men" encompasses diverse relations, identities, and practices that result in specific uses of the test as a prevention strategy. Thus, dialogue between programs, health professionals, and the persons most affected by the epidemic is crucial for building responses with real potential to confront the HIV epidemic, based on respect for human rights.

IntroductionGlobally, few trans women have access to transition-related care (TRC). Lack of TRC can lead to HIV risk as trans women may use sex to meet their needs for gender affirmation. Transwomen ...may also use non-prescribed hormones (NPH). Little is known about NPH use among transwomen in Brazil. We assessed the prevalence of NPH use and associated factors among transwomen (self-identified as trans, transsexuals and travestis) in 7 municipalities of São Paulo state in Brazil.MethodsMuriel Project was a cross-sectional consecutive snowball sampling study of 673 trans people, with face-to-face interviews carried out from November 2014 to October 2015. This analysis focuses on transwomen (n=616). Socio-demographics, stigma, health care access, transition procedures and self-reported HIV infection were assessed. Descriptive statistics and Poisson regression were performed to determine what factors were associated with use of NPH.ResultsNearly all transwomen (90.7%, n=559) reported ever taking hormones. The mean age for starting hormones was 17. Most (79.2%; n=444) participants reported using NPH, which was positively associated with sex work (p<0.005) and filler use (p<0.005). Chosen name use in public health services (p<0.05), gender confirmation surgery (p<0.05) and higher income (p<0.05) were all protective factors for NPH use.ConclusionTranswomen in Brazil likely have significant barriers to TRC, as evidenced by the high rate of NPH use in our sample, which may be underestimated given our sample was recruited from health care services. Stigma and few trans-specific health services may explain high use of NPH. Transwomen may have also used NPH and fillers to reach their transition goals faster. Transwomen engaged in sex work may rely on a feminised appearance for income, requiring the use of NPH in the absence of access to TRC. Ensuring social rights, such as requiring health providers and others use trans women’s chosen name, and providing adequate TRC services may reduce barriers to care, resulting in better health outcomes and less HIV risk for transwomen.

Resumen: Realizamos una revisión crítica de la literatura sobre el uso recurrente del test del VIH en hombres que practican sexo con hombres (HSH). Se realizó una revisión narrativa de la literatura analizando las diversas concepciones sobre los testes frecuentes a lo largo del tiempo, las implicaciones para los programas de salud y los principales marcadores sociales que influyen en la incorporación del test como atención de rutina. Aunque ha existido desde los años 1990, testes recurrentes entre HSH fueron frecuentemente interpretados como una mayor exposición al VIH debido a la falta de uso del condón, y por lo tanto como testes “innecesarios”. A partir de los años 2000, lo testes periódicos se han convertido en una recomendación y han sido interpretadas como una meta. La percepción de las personas sobre el uso que hicieron del test raramente fue considerada para caracterizar este uso como rutina de la atención. En el plano social y cultural, los aspectos individuales relacionados con los testes recientes o de rutina se incluyeron en contextos de normas favorables para las pruebas y disminución del estigma del SIDA. Las diferencias en la generación, la escolarización y los tipos de parejas afectivo-sexuales desempeñan un papel importante en las pruebas. Estas diferencias destacan que la categoría epidemiológica “hombres que tienen relaciones sexuales con hombres” abarca diversas relaciones, identidades y prácticas que resultan en usos específicos del test como estrategia preventiva. Por lo tanto, el diálogo entre los programas, los profesionales de la salud y las personas más afectadas por la epidemia del VIH es crucial para construir respuestas con el verdadero potencial para enfrentar la epidemia, sobre la base del respeto a los derechos humanos.

Introdução No contexto da valorização crescente do teste anti-HIV como estratégia de prevenção programática, a promoção do teste anti-HIV como estratégia de prevenção entre homens que fazem sexo com ...homens (HSH) é fundamental. Objetivo - Analisar os fatores associados tanto ao uso rotineiro como episódico do teste anti-HIV. Métodos - Os participantes foram 946 HSH entrevistados pelo Projeto SampaCentro em locais de sociabilidade HSH da região central de São Paulo entre novembro de 2011 e janeiro de 2012, nunca testados ou que procuraram o teste espontaneamente. A metodologia de amostragem foi a time-space-sampling e foram utilizados protocolos do Stata 12.0 para análise de amostras complexas. Os homens que se testaram por rotina ou episodicamente foram comparados aos nunca testados. As variáveis analisadas nos dois modelos de regressão de Poisson foram divididas em três níveis: características sociodemográficas (primeiro nível); socialização na comunidade gay e exposição da orientação sexual, discriminação e opiniões e atitudes em relação ao HIV/Aids e ao teste (segundo nível); percepção de risco, estratégias de prevenção e práticas e parcerias sexuais (terceiro nível). Resultados Os homens que se testaram rotineiramente eram mais velhos e moradores no Centro de SP. Além disso, tinham exposto a orientação sexual para profissional de saúde, sido discriminados em serviços de saúde mas não por amigos e/ou vizinhos (em razão da sexualidade) e não mencionaram medo do resultado do teste como motivo para HSH não se testarem. Também tinham maior probabilidade de conhecer pessoa soropositiva e de ter parcerias estáveis sem sexo anal desprotegido nas casuais (comparado a ter apenas parcerias casuais protegidas). Os homens que se testaram episodicamente eram mais velhos, residentes do Centro de SP, não moravam com parentes, expuseram sua orientação sexual para profissional de saúde, não reportaram medo do resultado do teste como barreira, conheciam pessoa soropositiva e mencionaram parceria estável sem sexo desprotegido com parceiro casual ou então sexo desprotegido em parcerias casuais (comparado a ter apenas parcerias casuais protegidas). Conclusões Os mais jovens, os que moram fora do centro de São Paulo, e os que expões menos sua orientação sexual são os segmentos que menos se testam rotineira ou episodicamente. Assim, dependem de ações para que seu direito seja protegido e assegurado. A estigmatização e a discriminação da homossexualidade deve ser combatida para que não impeça o acesso ao teste e a outros serviços de saúde. Disseminar informações e socializar os mais jovens para o diálogo sobre as estratégias de prevenção biomédicas e estratégias comunitárias de prevenção é necessário. Para ampliar o acesso e qualidade da testagem como recurso fundamental de programas de prevenção permanece o desafio de sustentar o debate sobre sexualidade e prevenção a cada geração, assim como nos programas de formação de educadores e de profissionais de saúde de todas as áreas.
Introduction The relevance of HIV testing is growing in programmatic policies. Promoting HIV testing among men who have sex with men (MSM) is of utter importance. Objective Analyze factors associated with routine and episodic use of HIV testing. Methods Our participants were selected from the database of Projeto SampaCentro, a time-location sampling serobehavioral surveillance survey of MSM in São Paulo, Brazil, that took place between November 2011 and January 2012. Our participants were 946 MSM not HIV positive who were never tested or whose last testing was client-initiated. All analysis were performed using complex samples protocols in Stata 12.0. Men were divided in routine testers, episodic testers and men who had never tested. Routine testers and episodic testers were compared to those who had never tested using two Poisson regression models. Variables were divided in three levels of analysis: sociodemographic (first level); socialization in gay community and disclosure of sexual orientation, discrimination, attitudes and opinions on HIV/AIDS and testing (second level); risk perception, prevention strategies and sexual practices and partnerships (third level). Results Routine testers were older and lived in Central São Paulo, had more frequently disclosed their sexual orientation to a health professional and been discriminated against in health service settings, were less likely to having suffered discrimination from friends and/or neighbors and to point fear as a barrier to testing, were more likely to know someone infected with HIV and to mention steady partners without unprotected anal intercourse (UAI) with casual partners (compared to having only casual partners with no UAI). Episodic testers were also older and more likely to live in Central São Paulo, less likely to live with relatives, more likely to have disclosed their sexual orientation to a health professional, less likely to point fear as a barrier to testing, more likely to know someone infected with HIV. This group more frequently mentioned steady partners without UAI with casual partners and UAI with casual partners, regardless of mentioning steady partners (compared to having only casual partners with no events of UAI). Conclusion Young MSM, those who live outside Central São Paulo and those less ready to disclose their sexual orientation are the MSM segments that are less likely to test, routinely or episodically. Therefore, programmatic actions are needed to ensure their rights are protected. Stigmatization and discrimination of homosexuality must be mitigated so that it wont be a barrier to information about prevention and testing services. Additionally, information on biomedical and communitarian prevention technologies must be shared and presented to those not yet familiar with these strategies. In order to improve access and quality of testing, the challenge of sustaining the debate about sex, sexuality and prevention remains, as new generations of MSM will need to be introduced to this debate. The same is true for the course curriculum of health professionals and educators.
