Abstract
This second part on Doppler sonography in prenatal medicine and obstetrics reviews its clinical applications. While this has not become the initially anticipated screening tool, it is used ...for the diagnosis and surveillance of a variety of fetal pathologies. For example, the sonography-based determination of uterine artery blood flow indices is an important parameter for the first trimester multimodal preeclampsia risk assessment, increasing accuracy and providing indication for the prophylactic treatment with aspirin. It also has significant implications for the diagnosis and surveillance of growth-restricted fetuses in the second and third trimesters through Doppler-sonographic analysis of umbilical artery, middle cerebral artery and ductus venosus. Here, especially the hemodynamics of the ductus venosus provides a critical criterium for birth management of severe, early-onset FGR before 34 + 0 weeks of gestation. Further, determination of maximum blood flow velocity of the middle cerebral artery is a central parameter in fetal diagnosis of anemia which has been significantly improved by this analysis. However, it is important to note that the mentioned improvements can only be achieved through highest methodological quality. Importantly, all these analyses are also applied to twins and higher order multiples. Here, for the differential diagnosis of specific complications such as TTTS, TAPS and TRAP, the application of Doppler sonography has become indispensable. To conclude, the successful application of Doppler sonography requires both exact methodology and precise pathophysiological interpretation of the data.

Im 2. Teil zum Einsatz der Methode der Dopplersonografie in der Pränatal- und Geburtsmedizin geht es um den klinischen Einsatz. Die Erwartungen einer breiten Anwendung der Methode als Screeningverfahren haben sich nicht bestätigt. Allerdings spielen die Widerstandsindizes der Aa. uterinae im Rahmen eines multimodalen Ansatzes für die Vorhersage der Präeklampsie im 1. Trimester eine wichtige Rolle, verbessern die Genauigkeit und schaffen die Grundlage für eine effektive Prophylaxe mit Aspirin. Für die Zustandsdiagnostik und Überwachung wachstumsretardierter Föten im 2. und 3. Trimester hat die Dopplersonografie der A. umbilicalis, A. cerebri media und des Ductus venosus eine herausragende Bedeutung. Insbesondere die Hämodynamik des Ductus venosus ist ein entscheidendes Kriterium für das Geburtsmanagement der frühen, schweren IUGR vor 34 SSW. Die maximale Geschwindigkeit in der A. cerebri media ist der zentrale Parameter in der fetalen Anämiediagnostik und hat deren Qualität grundlegend verändert und verbessert. Allerdings sind diese Verbesserungen an eine hohe methodische Qualität der Messung gekoppelt. Dies alles trifft natürlich auch für Zwillinge und höhergradige Mehrlinge zu. Die differenzierte Diagnostik spezifischer Komplikationen von monochorialen Mehrlingen wie FFTS, TAPS und TRAP ist ohne den Einsatz der Dopplersonografie nicht mehr zu denken. Der Erfolg des vielfältigen Einsatzes der Dopplersonografie ist verknüpft mit exakter methodischer Anwendung und präziser pathophysiologischer Interpretation.
Abstract
First-trimester screening between 11 + 0 and 13 + 6 weeks with qualified prenatal counseling, detailed ultrasound, biochemical markers and maternal factors has become the basis for decisions ...about further examinations. It detects numerous structural and genetic anomalies. The inclusion of uterine artery Doppler and PlGF screens for preeclampsia and fetal growth restriction. Low-dose aspirin significantly reduces the prevalence of severe preterm eclampsia. Cut-off values define groups of high, intermediate and low probability. Prenatal counseling uses detection and false-positive rates to work out the individual need profile and the corresponding decision: no further diagnosis/screening – cell-free DNA screening – diagnostic procedure and genetic analysis. In pre-test counseling it must be recognized that the prevalence of trisomy 21, 18 or 13 is low in younger women, as in submicroscopic anomalies in every maternal age. Even with high specificities, the positive predictive values of screening tests for rare anomalies are low. In the general population trisomies and sex chromosome aneuploidies account for approximately 70 % of anomalies recognizable by conventional genetic analysis. Screen positive results of cfDNA tests have to be proven by diagnostic procedure and genetic diagnosis. In cases of inconclusive results a higher rate of genetic anomalies is detected. Procedure-related fetal loss rates after chorionic biopsy and amniocentesis performed by experts are lower than 1 to 2 in 1000. Counseling should include the possible detection of submicroscopic anomalies by comparative genomic hybridization (array-CGH). At present, existing studies about screening for microdeletions and duplications do not provide reliable data to calculate sensitivities, false-positive rates and positive predictive values.
Zusammenfassung
Das Ersttrimester-Screening zwischen 11 + 0 und 13 + 6 Wochen mit qualifizierter Beratung, differenzierter Organdiagnostik sowie maternalen und biochemischen Markern ist die Grundlage der Entscheidung über den Umfang weiterer Untersuchungen. Mehr als die Hälfte relevanter fetaler Fehlbildungen können frühzeitig erkannt werden. Erhöhte Nackentransparenz und/oder auffällige biochemische Parameter weisen auf genetische oder strukturelle Anomalien hin. Durch Einschluss uteriner Dopplerparameter und des PlGF können die Risiken von Präeklampsie und Wachstumsrestriktion bestimmt und mittels der Gabe von ASS der weitere Verlauf zahlreicher Schwangerschaften positiv beeinflusst werden. Schwellenwerte (Cut-offs) und die Bildung von Bereichen hoher, intermediärer oder geringer Wahrscheinlichkeiten für das Vorliegen genetischer Anomalien dienen der Erläuterung der Erkennungs- und der Falsch-positiv-Raten. In der Beratung muss das individuelle Bedürfnisprofil der Schwangeren für entsprechendes Vorgehen (keine weitere Abklärung – Screening an zellfreier DNA – diagnostische Punktion) ermittelt werden. Die Beratung beinhaltet, dass in Kollektiven jüngerer Schwangerer und altersbedingt geringer Prävalenz oder beim Screening auf seltene submikroskopische Strukturanomalien auch bei hoher Spezifität der positive prädiktive Wert des Screenings gering ist. Innerhalb der Gesamtpopulation machen Trisomien und Anomalien der Geschlechtschromosomen etwa 70 % der lichtmikroskopisch erkennbaren Anomalien aus. Nach einem positiven Screening-Befund ist eine Absicherung durch diagnostische Punktion unerlässlich. Bei Testversagen besteht eine höhere Rate pathologischer Befunde. Die Verlustraten nach diagnostischen Punktionen liegen in Expertenhand um 1 bis 2 auf 1000 über der natürlichen Verlustrate. Die Beratung sollte die Möglichkeiten der Erkennung submikroskopischer Strukturanomalien mittels vergleichender genomischer Hybridisierung (Array-CGH) beinhalten. Belastbare Daten zu Sensitivität, Falsch-positiv-Raten und positiven prädiktiven Werten beim Screening auf Mikrodeletionen und -duplikationen lassen sich aus den bislang vorliegenden Studien nicht berechnen.
Abstract
Doppler sonography is widely established in prenatal medicine and obstetrics and is commonly used in screening for aneuploidy and preeclampsia during the first trimester. More importantly, ...during the 2nd and 3rd trimester, it is used in the assessment of fetal health as well as the surveillance of underlying fetal conditions such as IUGR and anemia. Correct use of the method is vital for correct data interpretation and the inferred clinical decision process. Therefore, we aim to provide all users of doppler sonography with a guide for meeting the required quality standards. These quality standards will further be the basis of DEGUM-certified training courses. In the first part, we will introduce the technical principles of the method and potential error sources, vascular disciplines to which the method can be applied, analysis of the spectral curves, patient safety and the needed requirements for successful certification.
Zusammenfassung
Der Einsatz der Methode der Dopplersonografie in der Pränatal- und Geburtsmedizin hat sich allgemein durchgesetzt. Sie wird beim Screening auf Aneuploidie und Präeklampsie im 1. Trimester zunehmend angewendet, aber vor allem zur fetalen Zustandsdiagnostik bzw. Überwachung gefährdeter Feten bei IUGR, Anämie und anderen Erkrankungen im 2. und 3. Trimester eingesetzt. Die korrekte Anwendung der Methode der Dopplersonografie ist die Voraussetzung für die richtige Interpretation der Ergebnisse und daraus abgeleiteter klinischer Entscheidungen. Diese Qualitätsanforderungen sollen hiermit für alle Anwender formuliert werden und gleichzeitig als Grundlage für die durch die DEGUM zertifizierten Kurse dienen. Im 1. Teil werden die allgemeinen technischen Grundlagen, Gefäßgebiete, Auswertung der Spektralkurven, Fehlerquellen, Patientensicherheit und die Voraussetzungen für eine erfolgreiche Zertifizierung behandelt.

Methoden Es wurde zunächst eine systematische Suche nach Leitlinien durchgeführt. Die aufgefundenen Leitlinien wurden mithilfe des DELPHI-Instruments der AWMF bewertet. Es wurden 3
LL für eine Adaptation für das CTG als grundsätzlich geeignet befunden. Es wurden 2 DEGUM Best Practice Guidelines für die Methodenbeschreibung als grundsätzlich geeignet befunden.
Zusätzlich wurden alle Studien zur Fragestellung anhand von 8 PICO-Fragen untersucht. Die strukturierte Konsensfindung zwischen den Fachgesellschaften erfolgte im Rahmen eines nominalen
Gruppenprozesses und einer strukturierten Konsenskonferenz unter unabhängiger Moderation.

Summary
Background
Immigration plays a major role in obstetrics in Austria, and about 18 % of the Austrian population are immigrants. Therefore, we aimed to (1) test the feasibility of a proposed ...questionnaire for assessment of migrant status in epidemiological research and (2) assess some important associations between procedures and outcomes in obstetrics and migration in selected departments in Austria.
Methods
We adapted a standardized questionnaire to the main immigration groups in Austria. Information on country of origin, length of residence in Austria and German-language ability was collected from eight selected obstetrics departments. Of the 1,971 questionnaires, 1,873 questionnaires of singleton births were selected and included in the analysis.
Results
We analyzed a total of 1,873 parturients with singleton births, of which 35 % had migrant status, 12 % were from ex-Yugoslavia, 12 % were from Turkey, and 12 % were from other countries. The proportion of parturients having their first care visit after the 12th week of pregnancy was higher in migrant groups (19 %). Smoking was highest in the migrants from ex-Yugoslavia (21 %). Vaginal delivery was more frequent in migrants from ex-Yugoslavia (78 %) and Turkey (83 %) than in nonmigrants (71 %) and episiotomy was more frequently performed in migrants from other countries. All differences are statistically significant.
Conclusions
Administration of a standardized questionnaire for assessment of migrant status in obstetric departments in Austria was shown to be feasible. We assessed differences in obstetric care and outcome and consequently recommend that action should be initiated in Austria toward harmonizing obstetric procedures among the migrant and the nonmigrant groups and toward minimizing risk factors.
