The title compound was synthesized by a Fischer ...indole reaction from 8-chloro-3,4-dihydro-1H-1-benzazepin-2,5-dione and 4-bromophenylhydrazine. It was characterized for structural identity by elemental analysis and spectroscopic methods (IR, NMR, EI-MS) and checked for purity by HPLC.
The intestinal immune system interacts with commensal microbiota to maintain gut homeostasis. Furthermore, stress alters the microbiome composition, leading to impaired brain function; yet how the ...intestinal immune system mediates these effects remains elusive. Here we report that colonic γδ T cells modulate behavioral vulnerability to chronic social stress via dectin-1 signaling. We show that reduction in specific Lactobacillus species, which are involved in T cell differentiation to protect the host immune system, contributes to stress-induced social-avoidance behavior, consistent with our observations in patients with depression. Stress-susceptible behaviors derive from increased differentiation in colonic interleukin (IL)-17-producing γδ T cells (γδ17 T cells) and their meningeal accumulation. These stress-susceptible cellular and behavioral phenotypes are causally mediated by dectin-1, an innate immune receptor expressed in γδ T cells. Our results highlight the previously unrecognized role of intestinal γδ17 T cells in the modulation of psychological stress responses and the importance of dectin-1 as a potential therapeutic target for the treatment of stress-induced behaviors.

Most patients receiving accelerated fractionation radiotherapy or chemoradiotherapy for head and neck cancer experience severe mucositis. This can lead to decreased oral intake, resulting in ...dehydration, severe malnutrition, hospitalization, and/or interruption of radiotherapy.
To evaluate the effect of prophylactic gastrostomy tubes (PGTs) on the rates of weight loss, unplanned interruptions, and hospitalization during high-intensity head and neck radiotherapy.
A retrospective review was performed on 88 patients treated for locally advanced head and neck cancer with accelerated twice-a-day radiation (n = 59) or concurrent chemoradiotherapy (n = 29). Prophylactic gastrostomy tubes were placed in 36 (41%) of patients in anticipation of increased acute toxic effects from treatment. The remaining patients without PGTs served as a control group.
Patients without PGTs lost an average 3.1 kg compared with 7.0 kg in the control group (P<.001). There were significantly fewer hospitalizations for nutritional or dehydration issues in those with PGTs than in the control group (13% vs 34%; P = .04, chi2 test). Among those with good performance status, no patient with a PGT required a treatment interruption, compared with 18% of patients without a PGT (P = .08). Sixteen patients (31%) in the control group underwent therapeutic gastrostomy tube placement during or after radiation therapy.
The use of PGTs significantly reduces weight loss and the rate of hospitalization for dehydration and complications of mucositis. Treatment interruptions may also be avoided by the use of PGTs in patients with good performance status. We encourage patients scheduled for intensive radiation therapy to receive a PGT.

Considerable research has examined depression and individual attitudes toward seeking professional psychological help, with recent research showing an emergent trend of increased depression among ...clergy. The McCall and Simmons role identity theory describes the dissonance professionals experience when personal standards, such as mental health are incongruent with professional standards like being a minister. When personal identity concerns such as depression are combined with the professional identity of ministers an internal conflict can occur. Baptist minister's levels of depression and their attitudes towards seeking psychological help were examined. Three hundred and forty nine ministers, randomly selected from a pool of 2,078 Baptist churches within a southwestern US state, agreed to participate in this study. The data were analyzed using a logistic regression to determine if demographic variables could predict ministers’ help seeking behaviors. Ministers reported a high rate of depressive symptoms, with 68.7% of the ministers having a depression score at or above the clinical cutoff score. The ministers' level of depression, type of ministry and years of education were significant predictors of ministers' attitudes towards seeking psychological help. The research findings are a catalyst for social change by providing an empirical investigation of the mental health needs of ministers. The results can assist practitioners with identification of the mental health needs of clergy members and support the need to identify evidence based interventions for ministers. Addressing and providing support for ministers' mental health needs will improve the quality of life for the ministers, their families, congregation and community.
