The pathogenic mechanism of hepatorenal syndrome is not well established. We investigated the circulatory function in cirrhosis before and after the development of hepatorenal syndrome. Systemic and ...hepatic hemodynamics and the activity of endogenous vasoactive systems were measured in 66 patients who had cirrhosis with tense ascites and normal serum creatinine levels; measurements were repeated at follow-up in 27 cases in whom hepatorenal syndrome had developed. At baseline, mean arterial pressure and cardiac output were significantly higher, and hepatic venous pressure gradient, plasma renin activity, and norepinephrine concentration were significantly lower in patients who did not develop hepatorenal syndrome compared with those presenting with this complication. Peripheral vascular resistance was decreased to the same extent in the two groups. Plasma renin activity and cardiac output were the only independent predictors of hepatorenal syndrome. Hepatorenal syndrome occurred in the setting of a significant reduction in mean arterial pressure (83 +/- 9 to 75 +/- 7 mmHg; P < .001), cardiac output (6.0 +/- 1.2 to 5.4 +/- 1.5 L/min; P < .01), and wedged pulmonary pressure (9.2 +/- 2.6 to 7.5 +/- 2.6 mmHg; P < .001) and an increase in plasma renin activity (9.9 +/- 5.2 to 17.5 +/- 11.4 ng/mL . hr; P < .001), norepinephrine concentration (571 +/- 241 to 965 +/- 502 pg/mL; P < .001), and hepatic venous pressure gradient. No changes were observed in peripheral vascular resistance. In conclusion, these data indicate that hepatorenal syndrome is the result of a decrease in cardiac output in the setting of a severe arterial vasodilation.
Introducción El síndrome de intestino irritable (SII) se ha considerado un trastorno funcional (no orgánico). Sin embargo, se maneja cada vez más la hipótesis de la existencia de fenómenos ...microinflamatorios intestinales. Objetivos Analizar la frecuencia de fenómenos inflamatorios en las biopsias colónicas de un grupo de pacientes diagnosticados de SII. Pacientes y métodos Se ha realizado un estudio prospectivo en 51 pacientes consecutivos con síntomas gastrointestinales bajos. Todos ellos cumplían los criterios de Roma II o III. El SII se clasificó como de predominio diarrea (SII-D), estreñimiento (SII-E) o alternante (SII-A). A todos se les realizó colonoscopia y biopsias a nivel de ciego y colon transverso. Se definió inflamación crónica inespecífica (ICI) ante la evidencia de un aumento de infiltrado de células plasmáticas y linfocitos en la lámina propia sin cumplir criterios de otro tipo de colitis. Se analizaron los datos mediante SPSS 12.0 utilizando el test de Chi cuadrado para el análisis estadístico. Resultados La edad media fue de 45,5 años±12,4 (17-75), siendo 11 (21,6%) varones y 40 (78,4%) mujeres. 26 pacientes (49%) no tenían patología asociada, 5 (9,8%) tenia diabetes mellitus, 6 (11,8%) depresión, 5 (9,8%) patología tiroidea, 3 (5,9%) fibromialgia y 6 (13,7%) otras patologías. El tiempo de evolución de los síntomas fue de 25,4±12,0 meses (3–120). 30 pacientes (58,8%) tenían síntomas diarios. 33 (64,7%) pacientes tenían SII-D, 11 (21,6%) SII-E y 7 (13,7%) SII-A. El SII-D fue el subtipo mas frecuente entre los hombres (10 (90,9%) hombres y 23 (57.5%) mujeres), pero sin alcanzar esta diferencia significación estadística (p=0,04). La mayoría de los pacientes (72,5%) no asociaban otra patología funcional, aunque 12 (23,5%) si tenían dispepsia funcional. En 35 pacientes se estudió la existencia de enfermedad celiaca con resultado positivo en 2 (5,7%). La colonoscopia fue normal en 39 pacientes (76,5%), se evidenciaron pólipos en 6 (11,8%), divertículos 3 (5,9%), melanosis coli 2 (3,9%) y fisura anal en 1 (2%). La mayor parte de los pacientes con SII (64,7%) tenían inflamación en las biopsias colónicas, siendo la ICI el hallazgo más frecuente (56,9%). Los diferentes cambios histológicos se muestran en la tabla adjunta. No se evidenciaron diferencias significativas entre los distintos grupos de SII y el hallazgo de ICI (p=0,569). La inflamación crónica de la mucosa del colon es frecuente en el SII con similar distribución en los distintos subtipos de SII. 2. Es preciso descartar la existencia de colitis microscópica y enfermedad celiaca en el subtipo SII-D ya que sus síntomas pueden ser confundidos.
Objetivo La combinación de un inhibidor de la bomba de protones (IBP), claritromicina y amoxicilina (IBP+C+A) es ampliamente utilizada para tratar la infección por H. pylori , aunque fracasa en un ...considerable número de pacientes. En dichos casos habitualmente se recomienda una terapia cuádruple con un IBP, bismuto, tetraciclina y metronidazol (IBP+B+T+M), aunque ésta también fracasa en3 20% de los casos. Nuestro objetivo fue evaluar la eficacia y la seguridad de un régimen basado en levofloxacino tras dos fracasos consecutivos en la erradicación de H. pylori , presentando los resultados actualizados de un estudio multicéntrico de la Asociación Española de Gastroenterología. Métodos Diseño : Estudio multicéntrico prospectivo. Pacientes : Pacientes consecutivos en los que había fracasado un primer tratamiento con IBP+C+A y un segundo con IBP+B+T+M (o ranitidina citrato de bismuto junto con esos mismos antibióticos). Criterios de exclusión : cirugía gástrica previa. Intervención : se administró un tercer tratamiento erradicador con levofloxacino (500 mg/12 h), amoxicilina (1 g/12 h) y omeprazol (20 mg/12 h) durante 10 días. Todos los fármacos se administraron juntos, después del desayuno y de la cena. El cumplimiento se evaluó mediante interrogatorio y recuperación de la medicación residual. Variable de resultado : la erradicación de H. pylori se definió como la negatividad de la prueba del aliento mediante13 C-urea 8 semanas después de finalizar el tratamiento. Resultados Se han incluido hasta el momento 171 pacientes, 14 (8%) de los cuales no finalizaron el seguimiento. Las tasas de erradicación, “por protocolo” y “por intención de tratar”, fueron del 71% (IC 95%=63–78%) y del 67% ((IC 95%=60–75%), respectivamente. Se describieron efectos adversos en el 21% de los casos, incluyendo náuseas (7%), sabor metálico (7%), mialgias/artralgias (7%), dolor abdominal (5%) y diarrea (4%), pero ninguno de ellos fue grave. Conclusión El tratamiento de “rescate” con levofloxacino constituye una prometedora alternativa de tercera línea tras el fracaso de múltiples terapias erradicadoras conteniendo antibióticos clave como amoxicilina, claritromicina, metronidazol y tetraciclina.
Quadruple therapy is generally recommended as second-line therapy after Helicobacter pylori (H. pylori) eradication failure. However, this regimen requires the administration of four drugs with a ...complex scheme, is associated with a relatively high incidence of adverse effects, and bismuth salts are not available worldwide anymore. Our aim was to evaluate the efficacy and tolerability of a triple second-line levofloxacin-based regimen in patients with H. pylori eradication failure.

in whom a first treatment with proton pump inhibitor-clarithromycin-amoxicillin had failed.
A second eradication regimen with levofloxacin (500 mg b.i.d.), amoxicillin (1 g b.i.d.), and omeprazole (20 mg b.i.d.) was prescribed for 10 days.
Eradication was confirmed with (13)C-urea breath test 4-8 wk after therapy. Compliance with therapy was determined from the interview and the recovery of empty envelopes of medications. Incidence of adverse effects was evaluated by means of a specific questionnaire.
Three hundred consecutive patients were included. Mean age was 48 yr, 47% were male, 38% had peptic ulcer, and 62% functional dyspepsia. Almost all (97%) patients took all the medications correctly. Per-protocol and intention-to-treat eradication rates were 81% (95% CI 77-86%) and 77% (73-82%). Adverse effects were reported in 22% of the patients, mainly including nausea (8%), metallic taste (5%), abdominal pain (3%), and myalgias (3%); none of them were severe.
Ten-day levofloxacin-based rescue therapy constitutes an encouraging second-line strategy, representing an alternative to quadruple therapy in patients with previous proton pump inhibitor-clarithromycin-amoxicillin failure, being simple and safe.
The known methods of transition fault diagnosis usually suffer from the drawback of many candidates. The method presented in this paper aims at reducing the number of suspects. The transition fault ...patterns were generated by Philips in-house ATPG tool and applied on the tester. The fail information from tester was subjected to fault diagnosis resulting in a small list of faulty candidates. We then injected the delay faults into the golden netlist of the test chip and confirmed through simulation whether or not their behavior matched with the tester results. Upon successful matching, we proceeded with the selection of few testable paths through the suspect faulty node and created corresponding path delay patterns using the path delay ATPG (a prototype at the University of Bremen, developed in cooperation with Philips Semiconductors GmbH, Hamburg). Finally, we verified those path delay patterns on the tester to increase the confidence level of the diagnosis method The experimental results show the effectiveness of our novel approach for improving diagnostic resolution.
