    原発性副甲状腺機能亢進症のうち,家族性副甲状腺機能亢進症の割合は2-5%であり,CDC73遺伝子バリアントを有する疾患として副甲状腺機能亢進症顎骨腫瘍症候群,家族性孤発性副甲状腺機能亢進症が挙げられる。今回原発性副甲状腺機能亢進症のため紹介され手術を施行したCDC73遺伝子バリアントを伴う副甲状腺癌の症例を経験したため報告する。症例は19歳女性,大腿骨頸部骨折を契機に原発性副甲状腺機能亢進症が疑われ当科受診。血液検査でカルシウム,intact-PTHが高値であり,画像検査で左下副甲状腺腫瘍及び全身の線維性骨炎を認めた。遺伝子検査でCDC73遺伝子バリアントが判明,左下副甲状腺及び甲状腺左葉合併切除を施行した。術後高カルシウム血症や線維性骨炎は改善,病理検査で副甲状腺癌の診断が確定した。今後も副甲状腺癌の再発転移やCDC73遺伝子バリアントに伴う全身合併症に注意し経過観察の方針である。
