Nous relevons l'importance primordiale du corps, celle des liens et de l'attachement, la nécessité de contextualiser le trauma, en incluant tant des données sociétales, statistiques, anthropologiques ...de santé publique qu'une perspective féministe. Abstract This article aims to shed light on how systemic approaches and psychotraumatology mutually enrich each other, leading to improvements in both the care patients receive and the systems in which they receive it. Les prises en charge qui ne tiennent pas suffisamment compte de ces éléments risquent de rajouter non seulement du trauma pour l'individu, mais aussi des dysfonctionnements familiaux dont les conséquences sur la famille actuelle, sur la structure du vivre ensemble de la société et aussi sur les générations futures peuvent se révéler désastreuses. Notre champ clinique de la thérapie familiale se doit de s'enrichir des apports de la psychotraumatologie afin d'être un acteur majeur dans le traitement et la prévention des conséquences des traumatismes car ces traumatismes naissent pour la grande majorité dans les liens familiaux.
Das Schutzprinzip beschreibt die Logik, durch die das psychische Geschehen bestimmt wird, wenn – jenseits des Lustprinzips – nicht mehr Konflikte, sondern Traumata die unbewusste Dynamik dominieren. ...Das Trauma beschädigt und durchschlägt symbolische Strukturen und schreibt sich ins Körperselbst ein. Die Erinnerung an diese Beschädigung manifestiert sich in einer permanenten Desorganisation des Körperselbst und geht mit einer Vernichtungsdrohung einher, die das Subjekt durch Versteinerung und ein systematisches Ausblenden der Wahrnehmung des Körpers abzuwehren sucht (verkapseltes Körperengramm). Die psychische Aktivität sucht sich vor einer Begegnung mit der Erinnerung an das Trauma zu schützen. Verschiedene defensive Strategien bis hin zur Selbstmordneigung werden beschrieben. Sowohl das Gedächtnis als auch die Abwehr der Vernichtungsdrohung sind körperlich konstituiert und bilden das körperliche Unbewusste jenseits des symbolischen Unbewussten. Typische Abwehrformen werden beschrieben. Anhand eines Fallbeispiels wird die Arbeitsweise der somatischen Narration erläutert: Aktiv wird die Körperwahrnehmung des Patienten einbezogen und erforscht. Die traumatische Einsamkeit wird jetzt durch die spezifische Präsenz der Analytikerin aufgehoben. Eine zerstörungsfreie Begegnung mit dem traumatischen Material reduziert die Abwehrnotwendigkeiten. Es wird möglich, vom Schutzprinzip zum Lustprinzip zurückzukehren.
Pensar el problema de las violencias sexuales en sus variadas formas de presentación, tanto dentro del supuesto espacio protector de las familias como fuera de este, nos remite a enfocar la carga de ...sufrimiento insoportable y la devastación emocional que estas pueden producir. En el caso de las mujeres, la transversalidad del fenómeno de la violencia a nivel social y político, que atraviesa categorías como la edad, la clase social, la ubicación geográfica, la etnia de pertenencia, la religión a la que se adscriba, etc., ha sido el factor desencadenante de sucesivas movilizaciones masivas que han generado la llamada cuarta ola del feminismo (Fernández, 2021; Varela, 2019). Se trata de la vivencia de alerta femenina compartida entre las mujeres, temor constante que tiene consecuencias en distintas esferas, permitiendo incluir la violencia sexual como forma de control social y de fragilización subjetiva (Fridman, 2005, citada en Fridman, 2019). Es así como el fenómeno de la violencia sexual nos lleva a enfocar el papel fundamental de la operación de devaluación e invisibilización que realiza la cultura sobre grupos minorizados: "Para que la violencia, el golpe, la violación, el acoso, el ataque incestuoso existan, es necesario que una sociedad haya, previamente, inferiorizado, discriminado, fragilizado al grupo social -las mujeres, los niños, los ancianos, etc.- que es objeto de violencia. Desde los discursos emanados por la salud pública, se lo posiciona como una problemática a nivel mundial, constituyéndose como un fenómeno de alta complejidad y gravedad, tanto a nivel individual como familiar y social.
Abstract
As emerging countries, China, Russia, and South Africa are establishing and/or improving their trauma systems. China has recently established a trauma system named “the Chinese Regional ...Trauma Care System” and covered over 200 million populations. It includes paramedic-staffed pre-hospital care, in-hospital care in certified trauma centers, trauma registry, quality assurance, continuous improvement and ongoing coverage of the entire Chinese territory. The Russian trauma system was formed in the first decade of the twenty-first century. Pre-hospital care is region-based, with a regional coordination center that determines which team will go to the scene and the nearest hospital where the victim should be transported. Physician-staffed ambulances are organized according to three levels of trauma severity corresponding to three levels of trauma centers where in-hospital care is managed by a trauma team. No national trauma registry exists in Russia. Improvements to the Russian trauma system have been scheduled. There is no unified trauma system in South Africa, and trauma care is organized by public and private emergency medical service in each province. During the pre-hospital care, paramedics provide basic or advanced life support services and transport the patients to the nearest hospital because of the limited number of trauma centers. In-hospital care is inclusive with a limited number of accredited trauma centers. In-hospital care is managed by emergency medicine with multidisciplinary care by the various specialties. There is no national trauma registry in South Africa. The South African trauma system is facing multiple challenges. An increase in financial support, training for primary emergency trauma care, and coordination of private sector, need to be planned.
Introducción: el neumomediastino (NM) se define como la presencia de aire libre alrededor de las estructuras mediastínicas. Es un hallazgo radiológico poco frecuente y alarmante en el contexto del ...paciente politraumatizado y su adecuado estudio permite descartar lesiones potencialmente letales. Métodos: se presenta el caso de un paciente masculino de 43 años de edad, ingresado en el contexto de politraumatismo contundente. Al examen físico, se encontraba con enfisema subcutáneo y saturometría limítrofe, con hallazgo tomográfico de NM. Resultados: se practicó esofagogastroduodenoscopia, que reportó normalidad y al examen físico no presentó aumento del enfisema subcutáneo en el seguimiento, catalogando el caso como secundario a efecto Macklin. Posteriormente, el paciente fue egresado en adecuadas condiciones y sin repercusión durante su estancia hospitalaria. Conclusiones: el NM es una entidad poco frecuente, pero alarmante, que obliga al conocimiento de sus causas y estudio de las mismas para descartar lesiones potencialmente letales.
IntroductionHealthcare systems across the world have struggled as a result of the Covid-19 pandemic. Most specialties have redeployed their staff and resources to deal with the pandemic whilst ...ceasing their planned elective activity. However acute specialties such as Trauma and Orthopaedics still have a significant emergency caseload that must be safely managed, even in a pandemic. The aim of this study was to investigate the change in Orthopaedic Trauma caseload in a Trauma Unit and a Major Trauma Centre during a pandemic compared to pre-pandemic levels and the associated staffing requirements. MethodsThe data presented was collected from a Trauma Unit and a Major Trauma Centre in the United Kingdom. We compared the number of accident and emergency referrals, fracture clinic appointments, inpatient admissions and operations during the six weeks of the first lockdown from 23rd March to May 3, 2020, to the same time period in 2019. ResultsThe results showed that the orthopaedic trauma caseload was approximately half that of pre-pandemic levels, reducing by an average of 54.2%. ConclusionA significant orthopaedic trauma caseload still remains to be safely managed during a pandemic and appropriate resources must be allocated. Staff allocation must take into account sick cover and staff wellbeing. A proportion of staff may need to be ring fenced from redeployment to facilitate this.
