    Background. To verify the supposition that it is possible to evaluate the cervical incompetence with help of sonography of uterine cervix, and that based on more objective sonographyc assessment of ... cervix the reduction of percentage of cerclage is possible in the General Hospital Slovenj Gradec without increasing the number of premature births. Methods. In the group of 112 singleton pregnant women in 18th-23rd week of gestation with clinical suspicion for cervical incompetence or history of premature births in previouspregnancies, only in 16 cases indication for cerclage was established using the transvaginal sonography with our own criteria for cervical incompetence. Statistical significance of difference in prevalence of cerclageand premature births in years before and after introduction of ultrasound diagnostic of cervical incompetence was analysed. Results. Using sonographyc criteria for cervical incompetence we have reduced prevalence of cerclage at the Department of Gynecology and Obstetrics of our hospital from 4% in year 1996 to 1% in year 1998 (p < 0.005) without the increase of percentage of premature births. Conclusions. Results of the study confirm thatsonography following the described criteria for cervical incompetence successfully supplements clinical examination of cervix in assessment of incompetence in second trimester of pregnancy. By using these objective sonographyc criteria for cervical incompetence, prevalence of cerclage was statistically significantly reduced without the increase of percentage of premature births.
