    Background. A thorough understanding of the clinical and pathological factors that affect the outcome of soft tissue sarcomas is essential in formulating a treatment plan for patients with these ... tumors. Research on prognostic factors in soft tissue sarcomas is hampered by the enormous heterogeneity of these tumors, their relatively low incidence, and their unique and unpredictable behavior. Moreover, there is significant disagreement regarding their histogenesis and grading, as well as lack of consensus on the value of variousprognostic factors. The first aim of this study was to show that grade,size, and stage have different prognostic significance for overall survival and distant recurrence than for local recurrence in patients with soft tissue sarcomas. The second aim was to identify other specific independent clinical and pathological prognostic factors for the end points oflocal recurrence, distant recurrence, post-metastasis survival, and overall survival in these tumors. Patients and methods. We analyzed data from 444 patients with primary soft tissue sarcomas with different localizations treated and followed-up at the Centro di Riferimento Oncologico, Aviano (Italy) from January 1985 to January 1997. Data of the patients were collectedretrospectively and prospectively. Median follow-up time was 35.6 months. Median age of the patients was 53 years (range, 10 to 95 years). We analyzed prognostic value of age, sex, performance status (ECOGžWHO), hemoglobin value, localization, size, histopathologic type, grade (Broders), TNM stage (UICCžAJCC) for local recurrence, distant recurrence, post-metastasis survival and overall survival. We also analyzed the influence of local recurrence for distant metastasis, postmetastasis survival and overall survival, and the influence of distant metastasis for overall survival.
