    Introduction. In addition to ventricular tachycardia, there have been several reports of cardiomyopathy induced by ventricular ectopy. Radiofrequency ablation of arrhythmogenic foci could be ... curative. Methods. Among 66 patients (48M/18F aged 51.7 +/- 14 years, LVEF 51.7 +/- 9.6%) with right ventricular outflow ventricular ectopy with or without ventricular tachycardia who were successfully treated with RF ablation, 10 patients (8M/2F aged 58.8 +/- l0 years, LVEF 32.1 +/- 5.8%) had clinical dilative cardiomyopathy and 56 patients (40M/16F aged 50.4 +/- 15 years, LVEF 55.2 +/- 4.6%) had normal left ventricular function. All patients were treated equally with radiofrequency ablation until the abolishment of any right ventricular outflow tract ventricular ectopic activity. The patients were screened clinically and echocardiographically and followed-up for 2 years after the procedure. Results. There was a considerable clinical and LV function improvement in the subpopulation of patients with dilative cardiomyopathy after 2 years; the LVEF increased from 32.1 +/- 5.8% to 54.6 +/- 5.1 % (P < 0.001) compared to patients without structural heart disease, in whom it decreased from 55.2 +/- 4.6% to 54.6 +/- 4.5% (P = NS). Conclusion. In all of our patients with DCM, successful eradication of ventricular ectopy was accompanied by improvements in left ventricular function. It seems on the basis of our clinical experience that ventricular ectopy from the right ventricular outflowtract should be treated even in patients without ventricular tachycardia in order to improve left ventricular function and prevent the development of cardiomyopathy.
