  • Kakovost vodenja antikoagulacijskega zdravljenja

    BACKGROUND: Anticoagulant drugs are of vital importance in the treatment of thrombembolic diseases. Their anticoagulant effect is produced by blocking theactivation of coagulation system. For the ... last 50 years coumarins are the mainstay of oral anticoagulant therapy. There is an increase in number of patients receiving coumarins due to width treatment of patients with atrial fibrillation and undulation. Management of oral anticoagulation include laboratory and clinical monitoring. Despite careful monitoring, oral anticoagulation is associated with serious hemorrhagic and thrombotic complications. AIM: The aim of this study was to evaluate the quality of control and clinical outcomes of oral anticoagulant treatment in patients managed in our anticoagulation clinic. METHODS: Since May 2001, anticoagulation treatment of all patients in our anticoagulation clinic is managed by computer-based dosing programme. Data from computer-based dosing programme was collected from May 2001 till June 2005 and analysed. For the patients which during the study period did not attend our anticoagulant clinic, additional information was searched at the primary care physicians. RESULTS: During the study period, 7437 patients were treated in our anticoagulation clinic. They were 72 +- 13 years old. The primary indication for anticoagulation was atrial fibrillation and undulation (62,5%). The numberof initiation of anticoagulant therapy was increasing progressively during the study period. This increase was mainly due to increased number of patients with mechanical heart valves (hi2: p=0,02). Analysis of data has shown that during the study period monitoring of 46% patients was transferred to primary care physicians, in 19% of patients anticoagulant treatment was stopped, 26% of patients continued treatment in our anticoagulation clinic, 6%of patients died.
